    An overview of nurse practitioner and its management system in the United States

    The United States is the first country to develop nurse practitioner(NP)programs. Experience has shown that NP training is one of the effective ways to improve the accessibility of healthservices,and to achieve a hierarchical management of nursing staff. This article reviewed the NP practice in the United States and showed that NP can provide health care services with higher quality and lower total cost than doctors. We summarized NP management system including licensure,accreditation,certification and education and provided reference for the establishment and improvement of Chinese NP training and practice.

    In the United Kingdom,nurse practitioners have been involved in the community health centers,emergency departments,pediatrics,etc. Nurse practitioners have the right to prescribe medicines which improves the efficiency and the effectiveness of health services,and decreases social costs. This article introduced the history,current status,training requirements,management and evaluation of nurse
    practitioners in the United Kingdom. We suggested to explore the ways that nurse practitioners can be involved in,build up tailored training system,develop concrete management system,and expand the nurse practitioner resources.

The practice of NP shows great advantages in the quality,safety,patient satisfaction and cost-effectiveness of primary care and chronic disease management. This article summarized the history,work contents,certification,training,and effectiveness of NP in Canada. Besides,we analyzed the contents that community nurses can learn from the practice of Canada in our country,which provided a reference for the training of NP in China.

    Doctor of Nursing Practice(DNP) program in Duke University is among the best in nursing schools in the United States. The program aims to train nurse leaders in interdisciplinary health care teams and advanced practice registered nurses(APRNs) to improve systems of care,patient outcomes,quality,and safety.Online courses supplemented by comprehensive on-campus orientation provide opportunities for those who want to pursue a DNP while earning an advanced nursing practice specialty. Additionally,DNP mentor team is comprised of faculty who are actively engaged in research,scholarship and teaching with broad experience in advanced clinical practice,leadership and patient safety,to guide the DNP students and guarantee the academic quality. The purpose of this article is to introduce this program,analyze the advantages and existing problems in the development of the DNP program,and further to provide valuable insights for the development of DNP program in China.

    Objective To explore the effects of online learning and cross-registration credit. Methods Firstly,a teaching team was formed,and the teaching objectives,plans,and evaluation method were all developed. A total of 107 nursing undergraduates were selected. Those students can choose online cross-registration credit courses in surgical nursing. The students were assessed by the combination of formative evaluation and summative evaluation.Results The score of formative evaluation was (26.87±1.23). The score of summative evaluation was (51.11 ±6.92). The pass rate was 98.13%. Conclusion The online cross-registration credit project can promote the sharing of high-quality teaching resources between different universities,promotes the academic exchange and cooperation among teachers.
    The application of extracurricular activities in the cultivation of undergraduates’ innovative ability

    Objective To explore the effects of innovative ability training for nursing undergraduates. Methods Measures included building up research group according to students interests,building up professional innovative associations establishing bases with functions of teaching and practice,and implementing individualized cultivating plans. Results From 2016 to 2018,there were 77 scientific research projects were funded respectively. Nine papers were published and 5 patents were approved. Students won 18 awards in various innovative competitions both inside and outside the school. They completed 2 municipal level projects under the guidance of teachers. Conclusion The extracurricular activities can improve the innovative ability of nursing undergraduates.

    Objective To evaluate the effects of team-based case learning applied in undergraduate nursing
    students’ safety and occupational protective course. Methods A total of 278 students of Grade 2016 were recruited.
    Team-based case learning,through which students could learn the related knowledge and skills from the real cases.
    Students’ knowledge of occupational protection were compared before and after the intervention.Results At the end of
    course,the scores of knowledge of occupational protection were significantly higher than that of pre-course(P<0.05).
    The course examination score was (89.41±0.78).The course study experience score was (83.30±13.62). Conclusion
    The team-based case learning for undergraduate nursing students’ safety and occupational protective course can
    improve their knowledge and ability of occupational protection.
    Case analysis in the management of nursing adverse events

    Objective To explore the effects of case analysis on nursing adverse events management. Methods
    Adverse events in a hospital were classified according to the classification standards of nursing adverse events is
    sued by the National Hospital Management Institute. Those adverse nursing events were pooled together and similar
    cases from different departments were analyzed every month. Besides,case analysis was conducted every 3 months
    at the hospital level. Interventions were proposed to eliminate the risk factors. The incidence of adverse events was
    compared before and after the case analysis. Results After the case analysis,the incidence of nursing adverse
    events decreased from 0.115% to 0.066%(X2=18.651,P=0.001) . Conclusion Case analysis can effectively reduce the
    incidence of nursing adverse events by enhancing the awareness of risk prevention,promoting the improvement of
    hospital environment and system process.
    Development of post competency evaluation system for nursing staff in nursing homes

    Objective To develop the pre-job evaluation system based on competency for caregivers in nursing

    homes. Methods Preliminary pre-job evaluation system was built up according to the literature review and experts consultations. After two rounds of Delphi expert consultations,the evaluation system was finally completed. Results The response rates

     of the two rounds of expert consultation were 100%,and the authoritative coefficient was 0.95.

    The variation coefficients of the first,level index ranged from 0.085 to 0.113 and those for second-level index
    ranged from 0.085 to 0.130. The final evaluation system included 5 first-level indicators and 21 second-level indi
    cators. Conclusion The evaluation system of pre-job for caregivers in nursing homes has good reliability and va
    lidity,and can be used for the pre-job of caregivers in nursing homes.

    Delirium knowledge needs of health care professionals in the intensive care unit of hospitals in Ningxia

    Objective To investigate the delirium knowledge and needs of health care professionals in the intensive care unit of hospitals in Ningxia. Methods The health care professionals from 4 hospitals of Ningxia were selected by convenience sampling. These health care professionals were investigated by questionnaires to know their delirium knowledge,knowledge acquisition ways,and the ways they expected to get the knowledge. Results The mean score of delirium knowledge of health care professionals was (56.10 ±18.24). Logistic regression analysis showed that age and profession were the influencing factors of delirium knowledge,OR values were 1.113 and 4.547. Over 79.19% of the participants considered clinical experience as their main source of knowledge about ICU delirium. 69.13% of the participants had never received any education on ICU delirium. Conclusion Health professionals in hospitals of Ningxia have lower level of knowledge on ICU delirium and the knowledge they have is far beyond enough to meet their clinical needs. It is suggested to adopt the model of clinical practice together with school education to train health care professionals and the students. By this way,the ability of detecting delirium early can be greatly improved.
    The effects of staged antiemetic care on prevention of chemotherapy induced nausea and vomiting of cancer patients

    Objective To investigate the effects of staged antiemetic care on prevention of chemotherapy induced nausea and vomiting(CINV) of cancer patients. Methods A total of 120 patients with nausea,vomiting and retching caused by chemotherapy were chosen by convenience sampling,and were evenly divided into the experimental group (60 patients) and the control group (60 patients). Patients in the experimental group received staged antiemetic care including pre-treatment prevention,mid-term care,and followed assessment to manage the adverse reactions of chemotherapy in patients. Patients in the control group received routine care. Results After 4 cycles of chemotherapy,the frequency,severity and duration of nausea,vomiting and retching in the experimental group
    were improved greater than those in the control group(P<0.05). Conclusion The staged antiemetic care can effectively improve the quality of life of patients and reduce the occurrence of CINV.
    A survey of nurses’ knowledge of PICC related thrombosis prevention and treatment in oncology departments

    Objective To know nurses’ knowledge of peripherally inserted central catheter(PICC)-related thrombosis prevention and treatment in oncology departments. Methods From February to October 2018,a cross-sectional survey was conducted on 234 nurses from 7 tertiary general hospitals and 3 tertiary oncology hospitals in Henan Province. Results The average score of knowledge level of Oncology nurses in PICC-related thrombosis prevention and treatment was 57.45±17.00. The thrombosis teaching and related training were the main factors that influenced the knowledge level of nurses and two factors accounted for 29.4% of total variance. Conclusion The knowledge
    level of oncology nurses about PICC-related thrombosis prevention and treatment was on the middle level. Clinical teaching and professional training are the main factors that should be encouraged to improve the knowledge level of oncology nurses

    Objective To investigate the job satisfaction of peripheral insert central catheter (PICC) specialist

    nurses in Hunan province and to analyze the related influencing factors. Methods The questionnaire survey was
    conducted among 387 PICC specialist nurses who were qualified from second or tertiary hospitals in Hunan
    Province by using the Index of Work Satisfaction of Chinese version (IWS). Results The mean total score of IWS
    among PICC specialist nurses in Hunan province was (134.69 ±18.46), The job satisfaction of PICC specialist
    nurses varied due to different levels of health status,monthly income,and whether the hospital had the PICC outpatient department, which accounted for 13.6% of the total variance. Conclusion The job satisfaction of PICC
    specialist nurses in Hunan province was at the middle level. The nursing managers need to take corresponding
    measures to improve job satisfaction of nurses and to stabilize specialist nurses.

    The relationships among organizational identity, psychological capital and job performance of clinical nurses

    Objective To investigate the relationships among psychological capital,organizational identification and job performance of clinical nurses in a tertiary hospital. Methods Totally 242 clinical nurses from a tertiary hospital were investigated by using Organizational Identification Questionnaire,Psychological Capital Scale and Job Performance Questionnaire. Results The scores of nurses’ organizational identity,psychological capital and job performance were 3.98±0.66,3.64±0.50 and 3.71±0.43 respectively. The correlation coefficients among the three variables ranged from 0.351 to 0.628. Psychological capital played a mediating role between organizational identity and job performance (Z=3.13,P<0.01). Organizational identity and psychological capital jointly explained 48.5% of the total variation in job performance. Conclusion Organizational identity and psychological capital are the influencing factors of job performance. Organizational identity can not only directly affect job performance,but also indirectly affect psychological performance through psychological capital. Nursing managers should take measures to improve nurses’ organizational identity and psychological capital in order to improve their work performance.
    Peripherally inserted central catheter related venous thrombus of 1030 tumor patients

    Objective To investigate the peripherally inserted central catheter (PICC )related venous thrombus of
    tumor patients and to know the prognosis of those patients. Methods A cross-sectional survey was conducted
    among 1 030 tumor patients admitted to Fudan University Shanghai Cancer Center in May 2016 to investigate the
    PICC related venous thrombus and to follow up their prognosis for 70 days. Results The first month of PICC insertion was the period with highest rate of PICC related venous thrombus and the rate was 5.4%(56/1 030).
    Among those venous thrombus cases,asymptomatic thrombosis was the most common with rate of 76.8%(43/56).
    Thirty-nine patients recovered after treatment. Conclusion Most of the PICC related venous thrombus are asymptomatic. Potential risk factors should be identified and treated in time to improve the prognosis of the tumor patients.
