Objective A community-based management pathway of "family-community-hospital" was constructed to provide theoretical basis for chronic disease management of COPD. Methods By using "Plan-Do-Check-Act" as the theoretical framework we designed"family-community-hospital" pathway for COPD. Fifteen experts in the field of clinical diagnosis and treatment, clinical nursing, community nursing, nursing management and nursing education in Tianjin were selected and two rounds of questionnaire consultations were conducted. The final pathway was completed by combining expert opinions and the index threshold screening method. Results The authority coefficient of experts was 0.90. The response rates of two rounds consultation were respectively 93.75% and 100%. The optimal management pathway of COPD in family-community-hospital was consisted of 6 first-level indicators and 21 second-level indicators. The first-level indicators included assessment, plan, action, evaluation, variation and feedback. The coordination coefficients were respectively 0.16 and 0.14. The score of the importance was up 3.5 points. The coefficient of variation was among 0.053 and 0.137. Conclusion The optimal management pathway of COPD with"family-community-hospital" has been proved to be scientific.

The 12 professional nurses were specifically trained to manage and follow up the patients with chronic respiratory disease. Two parts of theoretical training and clinical practice were included in the training which lasted for 3 months. The contents included definition and assessment of asthma and COPD,treatment, medication, physical activity, safety issues, lifestyle, family non-invasive ventilation, and airway clearance therapy. In addition, contents related to health education skills such as powerpoint preparation, teaching skills, preparation of research proposal, instrument development and its psychometric properties evaluation were also covered. The professional nurses were required to be placed in the clinical practice for 6 months and complete management of 50 cases per month. After they were qualified after the training, the hospital presented certificates to them. These nurses joined in the respiratory clinics and wards to provide education of self-management,medication,psychological support, safety, and family oxygen treatment. This approach has achieved certain effects which improved patients' disease self-management level and promoted the development of respiratory specialized nursing.

Objective To explore the diet management experiences of young and middle-aged patients with type 2 diabetes. Methods We conducted semi-structured in-depth interviews among 12 type 2 diabetes patients, and used Colaizzi 7-step analysis method for data analysis. Results Four themes emerged from the data, including multiple negative emotions, being difficult to make diet decisions, feeling the difficulty of insisting on the diabetes diets, and the desire to rebuild confidence and cope positively. Conclusion Young and middle-aged patients with type 2 diabetes generally have many problems in diet management. Their negative experience of diet management can't be ignored. Educators, families and society need to build support networks and provide effective support to help patients overcome difficulties and increase the positive experience of diet management.

Objective To explore the application of a home management platform in patients with chronic obstructive pulmonary disease and the effect of health management intervention. Methods A total of 80 patients admitted to the department of respiratory medicine of a third-level first-class hospital in zhengzhou from February 2018 to January 2019, who were in stable condition after treatment and discharged to the community, were selected as the study subjects, and were divided into the experimental group and the control group according to the time sequence of discharge, with 40 patients in each group. The control group adopted routine follow-up management, while the experimental group adopted home management platform for home management. COPD Assessment Test(CAT) was used to compare the health status of patients in the two groups. The chronic self-management behavior scale was used to compare the self-management behavior of the two groups after 2 months of follow-up. The disease-related knowledge of COPD patients was compared between the two groups. Results The total score of health status of COPD patients in the experimental group was lower than that in the control group. After 2 months of intervention, the total scores of self-management behavior scale and all dimensions in the experimental group were higher than those in the control group. The mastery of disease-related knowledge was higher than that of the control group and the difference were statistically significant(P<0.05). Conclusion The application of family management platform in the family management of COPD patients is conducive to improving the self-management behavior of patients, and improving the quality of life and health status of patients.

To improve the quality of clinical nursing skills, national clinical nursing skill competition for teachers in nursing colleges was held. Totally 276 full-time and part-time teachers from 92 universities in 29 provinces participated in the competition. This article introduced the competition design guided by modern nursing concepts and advanced education concepts. We also analyzed the existing problems found in the competition involving insufficiencies of training in human care competencies, holistic care, clinical thinking, nursing communication, collaboration and theory application. In view of the existing problems, the author proposed measures of teaching reform of clinical nursing skills based on "Golden Course” proposed by the Ministry of Education.

Methods Forty-four new nurses in the operating room from 2017 to 2018 were trained.Twenty-three new nurses recruited in 2017 were assigned into the control group. The traditional training method was used in the control group including knowledge learning, video watching, and clinical practice. Twenty-one nurses recruited in 2018 were assigned into the experimental group and the defect situational teaching method was used. The work efficiency and accuracy of handover contents of the two groups were compared. Besides, the evaluation of the teaching method was investigated among the experimental group. Results The work efficiency and accuracy of handover contents of new nurses in the experimental group were higher than those in the control group(P<0.05). Nineteen new nurses in the experimental group were satisfied with the teaching method. Conclusion The defective situation teaching can promote the learning initiative and enthusiasm of the new nurses. It can help to improve the new nurses' safe handover process in operating room.

The status and influencing factors of evidence-based practice ability among emergency nurses
JIANG Yun, CUI Miao-ling, YING Yan-ping, YANG Li, HUANG Shuang-xia, ZHONG Juan
    2020, 17 (5):  469-472.  doi: 10.3761/j.issn.1672-9234.2020.05.019
    Abstract ( 383 )   HTML ( 0 )   PDF (562KB) ( 18 )   Save References | Related Articles | Metrics
Objective To understand the status of evidence-based practice ability of emergency nurses, and to explore its possible influencing factors. Methods By convenience sampling,a cross-sectional survey of 236 nurses from 46 hospitals in Guangxi was conducted using the Chinese version of Evidence Based Practice Evaluation Competence Questionnaire(EBP-COQ). Results The total average score of EBP-COQ of emergency nurses was(3.43±0.35),and the average scores of all dimensions were(3.77±0.37) for total,(3.34±0.49) for knowledge,and(3.17±0.47) for skill. Multiple linear regression analysis showed that educational background had significant effect on the total score of the EBP-COQ;educational background, and whether there was a certificate of emergency specialist nurses affected evidence-based practice attitudes;age affected evidence-based practice knowledge. In recent 5 years, the number of papers published as first author or corresponding author affected evidence-based practical skills. Conclusion At present, emergency nurses in Guangxi can treat the practice of evidence-based nursing positively, but the whole level of evidence-based practice ability needs to be improved. According to the weakness of evidence-based practice ability, the training plan can be formulated to improve the evidence-based practice ability of emergency nurses, and promote evidence-based practice in clinical practice all-round development.
