Abstract: Objective To explore the application of a home management platform in patients with chronic obstructive pulmonary disease and the effect of health management intervention. Methods A total of 80 patients admitted to the department of respiratory medicine of a third-level first-class hospital in zhengzhou from February 2018 to January 2019, who were in stable condition after treatment and discharged to the community, were selected as the study subjects, and were divided into the experimental group and the control group according to the time sequence of discharge, with 40 patients in each group. The control group adopted routine follow-up management, while the experimental group adopted home management platform for home management. COPD Assessment Test(CAT) was used to compare the health status of patients in the two groups. The chronic self-management behavior scale was used to compare the self-management behavior of the two groups after 2 months of follow-up. The disease-related knowledge of COPD patients was compared between the two groups. Results The total score of health status of COPD patients in the experimental group was lower than that in the control group. After 2 months of intervention, the total scores of self-management behavior scale and all dimensions in the experimental group were higher than those in the control group. The mastery of disease-related knowledge was higher than that of the control group and the difference were statistically significant(P<0.05). Conclusion The application of family management platform in the family management of COPD patients is conducive to improving the self-management behavior of patients, and improving the quality of life and health status of patients.
