三级综合医院护士安宁疗护实施困难现况调查研究

摘要：

目的调查护士实施安宁疗护的困难现状，为制订针对性管理策略提供参考。方法采用便利抽样法，于2023年8月—2024年5月选取福建省9个市27所三级综合医院的临床护士作为调查对象，采用一般资料调查表、汉化版安宁疗护实施困难量表进行调查。结果共发放2 128份问卷，其中有效问卷2 047份，问卷有效回收率为96.2%。护士安宁疗护实施困难得分为（46.29±8.44）分，各维度条目均分由高到低依次为社区协调（3.11±0.82）分、知识缺乏（3.08±0.77）分、与患者及家属的沟通（3.02±0.76）分、多学科团队的沟通（2.98±0.68）分。不同年龄、工作年限、文化程度、职称、婚姻情况、以往是否有照护终末期患者及其家属经历的护士安宁疗护实施困难得分比较，差异具有统计学意义（P<0.05）。结论护士安宁疗护实施困难水平较高，仍待进一步改善，建议管理者制订系统全面的安宁疗护继续教育体系，加强护士相关培训，以降低其安宁疗护实施困难水平。

Abstract:

Objective To investigate the current difficulties faced by nurses in implementing palliative care and provide references for formulating targeted management strategies. Methods A total of 2，047 clinical nurses from 27 tertiary general hospitals in 9 cities of Fujian Province were selected as the survey subjects from August 2023 to May 2024 by the convenience sampling method. The survey was conducted using the general information questionnaire and the Chinese version of the Palliative Care Difficulty Scale. Results A total of 2，128 questionnaires were distributed，among which 2，047 were valid，and the valid recovery rate of the questionnaires was 96.2%. The score of nurses’ difficulty in implementing palliative care was（46.29±8.44） points. The average scores of each dimension item from high to low were community coordination（3.11±0.82） points，lack of knowledge （3.08±0.77） points，communication with patients and their families（3.02±0.76） points，and communication with multidisciplinary teams（2.98±0.68） points. The comparison of the total scores of difficulties in implementing palliative care among nurses of different ages，working years，educational levels，professional titles，marital status，and whether they had previous experiences in caring for terminally ill patients and their families showed statistically significant differences（P<0.05）. Conclusion The difficulty level of nurses in implementing palliative care is relatively high and still needs to be further optimized. It is suggested that managers formulate a systematic and comprehensive continuing education system for palliative care and strengthen relevant training for nurses to reduce their difficulty level in implementing palliative care.
