Construction and application of a training program for vascular access nursing ability of blood purification refresher nurses

摘要:

目的 构建血液净化护士血管通路护理培训方案并验证其实施效果。方法 在文献回顾的基础上,以Miller金字塔理论的4个阶段为培训内容模块框架,形成方案初稿。选取19名专家开展2轮函询,构建最终方案。选取2023年1月—12月南京市某三级甲等医院血液净化中心40名进修护士作为对照组,2024年1月—12月40名进修护士作为试验组。对照组采用常规临床带教教师“一对一”带教模式,试验组实施基于Miller金字塔模型的培训方案,比较两组血管通路理论知识、知识应用能力、专业实践技能、临床情境模拟考核成绩,以及对培训的满意度。结果 培训后两组理论考核、知识应用能力得分差异无统计学意义(P>0.05),试验组专业实践技能、临床情境模拟考核得分高于对照组,差异有统计学意义(P<0.05)。试验组对该培训的总体满意度得分为高于对照组(P<0.05)。结论 基于Miller金字塔模型的血液净化血管通路护理培训方案可培养进修人员对血管通路的评估、使用、维护、并发症预防等护理能力,具有一定的规范性和科学性,未来还需进一步验证培训对护士实际工作表现的作用影响。

Abstract:

Objective To construct a standardized training program targeting the vascular access nursing competence of blood purification nurses and to empirically validate its effectiveness in clinical practice. Methods A preliminary program framework was established through systematic literature retrieval and analysis,with the four hierarchical stages of Miller’s Pyramid Theory serving as the core module for organizing training content. Nineteen interdisciplinary experts,including clinical nursing specialists,nursing educators,and vascular surgery physicians,were invited to conduct two rounds of Delphi correspondence consultations. The program was revised and finalized based on expert feedback regarding content validity,feasibility,and scientific rigor. A total of 80 refresher nurses rotating through the Blood Purification Center of a tertiary Grade A hospital in Nanjing were enrolled in this study. The control group(n=40) consisted of nurses who received conventional one-on-one clinical preceptorship from January to December 2023,while the experimental group(n=40) received the Miller’s Pyramid-based training program from January to December 2024. The two groups were compared in terms of theoretical knowledge of vascular access,knowledge application proficiency,hands-on practical skills evaluation,clinical scenario simulation testing,and satisfaction with the training. Results Post-training comparative analysis revealed no statistically significant difference in theoretical knowledge and knowledge application scores between the two groups(P>0.05). However,the experimental group demonstrated significantly higher performance in professional practical skills(P<0.05) and clinical scenario simulation assessment(P<0.05) compared to the control group. The overall satisfaction score of the experimental group was significantly higher than that of the control group(P<0.05). Conclusion The Miller’s Pyramid-based training program for vascular access nursing competence in blood purification effectively enhances refresher nurses’ capabilities in vascular access assessment,rational utilization,standardized maintenance,and complication prevention. Characterized by standardization and scientific rigor,the program provides a structured approach to nursing competence development. Future research should further explore the long-term impact of this training on nurses’ actual clinical performance and patient outcomes through longitudinal follow-up studies.
