    Construction of evaluation index system for core competence of nurses sent in advance in public health emergencies

    Objective To build a core competency evaluation index system for nurses sent in advance in public health emergencies. Methods The preliminary draft of core competency evaluation index system for nurses was developed. The Delphi technique was used to conduct two rounds of letter consultation with 26 experts with first-aid experience to determine the final index system. Results The recovery rate of the two rounds of expert letter questionnaire was 100%,and the coefficient of expert authority in the second round was 0.785. W value for first level index of the coordination coefficient was 0.594,W value for second-level indicators was 0.825,and W value for third-level indicators was 0.739. After two rounds of expert letter consultation,three first-level indicators including first-aid nursing skills,knowledge of infectious diseases,and emergency response ability,12 second-level indicators and 52 third-level indicators were finally determined. Conclusion The construction process of the core competency evaluation index system for nurses sent in advance in public health emergencies is scientific. The content is comprehensive,which can provide a basis for the emergency medical rescue team to screen the first batch of high-quality rescuers.

    Objective To explore the effects of teach-back in health education of older patients with esophageal cancer after operation. Methods A total of 72 older patients who underwent esophageal surgery in thoracic surgery from June to September 2020 of a tertiary hospital in Henan Province were randomly divided into the control group(n=35) and the experimental group(n=37). Patients in the control group was given routine education on naso-intestinal tube by oral education and video watching. Patients in the experimental group was given naso-intestinal health education by teach-back method,which mainly included explanation,evaluation,clarification,re-education and understanding. The unplanned extubation rate of naso-intestinal tube and the score of nursing knowledge after intervention were compared between the two groups. Results After the intervention,the unplanned extubation rate(0) of naso-intestinal tube in the experimental group was significantly lower than that of the control group(24.79‰)(P=0.010). The scores of nursing knowledge about naso-intestinal tube(82.70±9.32) was significantly higher than that of the control group(50.57±15.89)(t=10.388,P<0.05). Conclusion Teach-back method is a practical and effective health education method,which can improve the knowledge level of naso-intestinal nursing and reduce the incidence of unplanned extubation.

    Application of periodic assessment based on the forgetting curve in health education of patients after rotator cuff repair

    Objective To explore the effects of periodic assessment based on the forgetting curve in health education of patients undergoing rotator cuff repair. Methods One hundred and twenty patients who underwent arthroscopic rotator cuff repair in a tertiary hospital in Nanjing from June 2019 to July 2020 were randomly divided into the experimental group and the control group,with 60 cases in each group. The patients in the control group received routine care. Patients in the experimental group received a periodic assessment based on the forgetting curve besides routine care and re-education based on assessment results. The shoulder joint range of motion and Constant-Murley score were compared between the two groups 1 day before surgery,2 weeks and 3 months after surgery. The test score of patients in the experimental group was compared at 20 min,the day of surgery and 1 d,5 d,2 weeks,1 month and 3 months after surgery. Results There was an interaction effect of the shoulder range of motion between groups and time:the forward bending angle(Finteraction=18.044,P<0.001),the abduction angle(Finteraction=14.855,P<0.001),and the neutral position of external rotation angle(Finteraction=45.280,P<0.001). The simple effect analysis showed that after 2 weeks and 3 months of the surgery,the shoulder range of motion in the experimental group was greater than that in the control group,so was the forward bending angle(t=2.232,31.950,P<0.05),the abduction angle(t=32.950,31.260,P<0.05),the neutral position of external rotation angle(t=8.387,12.488,P<0.05). The Constant-Murley score had an interaction effect between groups and time(Finteraction=117.000,P<0.001). A simple effect analysis showed that after 2 weeks and 3 months of surgery,the score of the experimental group was higher than that of the control group(t=11.547,11.123,P<0.001). There are significant differences in the assessment scores of the experimental group at each stage(F=100.043,P<0.05),and the overall scores show an upward trend. Conclusion The health education method based on the forgetting curve theory is feasible. It helps to strengthen patient’s rehabilitation knowledge,improve the effects of health education,thereby improving health outcomes of patients after operation. The study provides a new way for clinical health education.

    The effects of pelvic floor muscle training based on audio guidance on postpartum pelvic floor rehabilitation in primiparas with stress urinary incontinence

    Objective To explore the effects of pelvic floor muscle training based on App-based audio guidance on postpartum pelvic floor rehabilitation in primiparas with stress urinary incontinence. Methods A total of 108 pregnant women with stress urinary incontinence were recruited and divided into the experimental group(n=54)and the control group(n=54) by lottery. Patients in the experimental group received audio guided exercises on the basis of routine care. Patients in both groups exercised to 6 months after childbirth. The pelvic floor muscle surface electromyography(sEMG),exercise compliance and the severity of stress urinary incontinence at 6~8 weeks,3 months,and 6 months after childbirth were compared between the two groups. Results At 6~8 weeks,3 months,and 6 months after childbirth,the compliance of the experimental group with pelvic floor muscle exercise was better than that of the control group(P<0.05). There was an interaction effect between groups and time in the average sEMG value of type Ⅰ fibers(F=4.702,P=0.011),and the simple effect analysis showed that the average sEMG value of type Ⅰ fibers in the experimental group were higher than those in the control group at different stages postpartum(P<0.05). The maximal sEMG value of type Ⅱ fibers was statistically significant between groups(F=13.881,P<0.001),and the time effects were statistically significant(F=228.685,P<0.001). The interaction between the invisible group and time was not statistically significant(F=0.664,P=0.517). There was an interaction effect between groups and time in the average sEMG values of endurance test(F=4.009,P=0.021).The simple effect analysis showed that at 6~8 weeks,3 months,and 6 months after childbirth,the average sEMG value of endurance test in the experimental group were higher than that in the control group(P<0.05).The severity of stress urinary incontinence in the two groups were relieved,however,the differences between the groups were not statistically significant(P>0.05). Conclusion Audio guidance is helpful to improve the compliance with the pelvic floor muscle training of pregnant women therefore improving postpartum pelvic floor muscle strength.

    Bibliometric analysis of empowerment in care of patients with diabetes mellitus in China

    Objective To analyze the research status and development trend of empowerment in care of patients with diabetes mellitus in China. Methods The literature related to the application of empowerment in care of patients with diabetes mellitus were searched in the database of CNKI,Wanfang and VIP. The retrieval period was from January 2008 to December 2019. The bibliometric analysis was conducted by Bicomb 2.0 software and SPSS 23.0 software to find the key words,research focus and trends of the included papers. Results A total of 139 related articles were obtained. Ten high-frequency key words were identified,and three research hot topics in this field were obtained after clustering analysis including correlation analysis of empowerment and blood glucose control,self-management in patients with diabetes mellitus,analysis of empowerment in patients with type 2 diabetes mellitus,and application of empowerment in diabetes health education promotion. Conclusion The application of empowerment in care of patients with diabetes mellitus has been paid more and more attention by researchers.It is suggested that the empowerment should be combined with various intervention modes,and the intervention sites and groups should be further expanded to promote the development of the theory of the empowerment according to the factors affecting the empowerment ability of diabetes patients.

    Objective To explore the effects of Narrative Nursing course on narrative competence and caring ability of nursing undergraduates. Methods A total of 96 nursing undergraduates were included in this study. The course of Narrative Nursing was carried out from September to December 2019. The course had a total of 18 hours. Before and after the course,the Narrative Competence Scale and Caring Ability Inventory were used to measure the narrative competence and caring ability of students respectively. At the end of the course,the semi-structured interview was conducted to deeply understand students’ learning feelings and suggestions for the course. Results Scores of the ability to listen was higher than the score before the course(P<0.001). There was no statistical difference between the total scores of narrative competence,the ability to understand,reflect and reproduce(P>0.05). Scores of caring ability were higher than scores before the course(P<0.001). The results of the interview included students’ learning experience and students’ expectations for the course, and the former included the improvement of narrative nursing ability, the deepening of understanding of humanistic care, and the re-understanding of life. Conclusion Narrative Nursing can improve the caring ability and the ability to listen of nursing undergraduates. Future researches are needed on other aspects of narrative competence.

    Online learning status of nursing undergraduates during the special period of “class suspension with learning uninterrupted”

    Objective To investigate the effects and influencing factors of online learning of nursing undergraduates during the epidemic period of COVID-19. Methods From June 10 to June 30,2020,totally 37,301 students from 150 nursing colleges in China were investigated. A self-designed questionnaire was used. Results During the epidemic period,the average number of online learning courses was(8.45±3.30),and the average learning time per day was(5.81±2.51) hours. The learning time per day during the epidemic period was less than that before the epidemic(t=2.342,P=0.019);the mainly online learning pattern was “PPT recording+online discussion”(51.9%),followed by “whole online live”(24.2%). The proportion of students participation in online experiment,online clinical practice and online examination were 81.3%,34.5% and 60.7% respectively. For the choice rate of learning styles,offline supplemented by online,online supplemented by offline,whole offline,whole online were 47.3%,25.0%,16.6% and 4.8% respectively. Conclusion The online teaching task of nursing undergraduate colleges and universities in China has been successfully completed by adopting various forms of online teaching,but the abilities of students’ online learning and teachers’ online teaching need to be improved.

    Objective To investigate students’ cognition and willingness of nurses’ multiple-sites practice. Methods A self-developed questionnaire was used to investigate 342 nursing students from 5 nursing colleges in Shanghai,and a semi-structured interview was conducted among 10 nursing students. Results A total of 275(80.4%) nursing students agreed with nurses’ multiple-sites practice,201(58.8%) nursing students had the willingness to practice in multiple-sites,192(56.1%) nursing students expressed optimistic or very optimistic attitude about the prospects of nurses’ multiple-sites practice. Totally 84(24.6%)nursing students indicated that they knew very well or understood that nurses’ multiple-sites practice more. The interview data showed that students had few ways to understand multiple-sites practice. The main reasons for nursing students’ concerns were incomplete laws and regulations. Conclusion Nursing students show a positive support and willingness to nurses’ multiple-sites practice. But a few people knew it well. It is hoped that schools,hospitals and other relevant departments can increase the contents and policy introduction of nurses’ multiple-sites practice. To improve students’ knowledge of nurses’ multiple-sites practice is conducive to better practice in the future,and further promote the diverse nursing development.

    The mediating effect of self-regulated learning ability between critical thinking and resilience of nursing undergraduates

    Objective To understand the mediating effect of self-regulated learning ability between psychological resilience and critical thinking ability of nursing undergraduates. Methods A total of 288 nursing undergraduates were recruited by convenience sampling and investigated by the nursing specialty self-regulated scale,California Critical Thinking Disposition Inventory-Chinese Version and the Resilience Scale of Adults. Results The scores of critical thinking ability,self-regulated learning ability and resilience were(231.89±29.03),(106.35±14.43) and (156.59±24.84) respectively. The total effect coefficient of critical thinking on resilience was 0.343. The indirect effect coefficient was 0.146 and 95%CI was 0.060~0.218. The direct effect coefficient was 0.197 and 95%CI was 0.163~0.230. Conclusion The critical thinking,self-regulated learning ability and resilience of nursing undergraduates were in the middle level. There is a mediating role of self-regulated learning ability between critical thinking and resilience. Teachers should optimize the blended curriculum,strengthen the clinical practice and interpersonal relationship to cultivate students’ critical thinking and improve the level of psychological resilience by influencing self-regulated learning ability.

    Objective To investigate the current situation of hospice care ability of undergraduate nursing students. Methods A survey of 296 undergraduate nursing students was conducted by using the hospice care ability questionnaire. Results The score of hospice care ability of undergraduate nursing students was(41.52±9.59),which was below the medium level,including(14.48±3.91) for effective nursing skills,(15.49±3.34) for cultural and ethical values,and(11.55±2.96) for cross-professional teamwork. The factors influencing students’ ability of hospice care were the place of origin,whether they had cared for the critically ill patients,and whether they had participated in the training related to hospice care accounting for 32.2% of total variance. Conclusion The knowledge level and nursing skills of the undergraduate nursing students in hospice care need to be improved,and students’ teamwork ability is also relatively weak. Nursing education and management should actively disseminate and popularize the idea of hospice care,and develop the program of hospice care ability cultivation for undergraduate nursing students as soon as possible,so as to provide valuable reference for promoting the development of hospice care education in nursing.

    Training and use of clinical nursing teachers in a general hospital

    Objective To explore the methods and effects of training and using clinical nursing teachers in a general hospital. Methods The special position for clinical nursing teachers was set up. The position requirements,responsibilities,role function,and remuneration were all clarified. The training program was developed based on the education procedure and provided to the 43 nurses. After qualification,these nurses were investigated by the self-developed questionnaires of workload,working ability and teaching quality. Results The annual workload score of 43 nurses was 88.73±1.82. After the training,the score of teaching ability was 85.63±2.26,which was higher than that before the training(74.51±2.18)(t=3.37,P=0.030). After the training and use of clinical nursing teachers,the score of teaching quality of 43 nursing units was 90.21±1.89,which was higher than that before the training(84.82±2.57)(t=2.66,P=0.040). Conclusion The standardized training,use and management of clinical nursing teachers are helpful to improve the teaching level and quality of the whole hospital and promote the development of clinical nursing teaching.

    Mediating effects of job crafting on working stressor and presenteeism of new nurses

    Objective To explore the influence of working stressor on presenteeism of new nurses,and to examine the mediating role of nurses’ job crafting. Methods A total of 430 new nurses were investigated with the General Information Questionnaire,the Stanford Presenteeism Scale,Challenge- and Hindrance-related Self-reported Stress Scale,and the Job Crafting Scale. Results The average scores of presenteeism,challenge stressor,hindrance stressor and the job crafting of new nurses were as follows:(2.95±0.74),(2.59±0.62),(2.69±0.72),and(2.85±0.50). There was a negative correlation between challenge stressor and presenteeism(r=-0.369,P<0.001),and a positive correlation with job crafting(r=0.393,P<0.001). While,there was a positive correlation between hindrance stressor and presenteeism(r=0.625,P<0.001),and a negative correlation between hindrance stressor and job crafting(r=-0.102,P=0.034). There was a partial mediating effect of job crafting between challenging stressor and presenteeism,and themediating effect(-0.034)accounted for 17.1% of the total effect(-0.199). Conclusion There was a high level of presenteeism among new nurses. Nursing managers can take targeted strategies to help new nurses dealing with stressor appropriately to promote their job crafting.

    Objective To examine the effects of honey on oral mucositis prevention in adults with malignant hematological disease undergoing chemotherapy. Methods In this prospective randomized clinical trial,98 patients with leukemia or lymphoma undergoing chemotherapy in three tertiary general hospitals from July 2018 to March 2019,were randomly allocated into two groups:the experimental group(n=49)and the control group(n=49). Patients in the control group were given routine oral care and alkaline mouthwash,while patients in the experimental group were given honey on the surface of the oral mucosa in addition to the routine care. The incidence,time,and severity of oral mucositis between the two groups were compared. Results The incidence of oral mucositis in the experimental group was significantly lower than that in the control group(χ2=5.288,P<0.05). There was no significant differences in the time of oral mucositis occurrence between the two groups(t=0.386,P=0.705). There was also no significant differences in oral mucositis severity in two groups(P>0.05). Conclusion Honey can reduce the incidence of chemotherapy-induced oral mucositis,and has a preventive effect on chemotherapy-induced oral mucositis.

    The perioperative safety management practice of patients with advanced age undergoing endoscopic retrograde cholangiopancreatography

    Objective To evaluate the effects of multi-disciplinary nursing project on perioperative safety management for patients aged 80 or above with endoscopic retrograde cholangiopancreatography(ERCP). Methods Totally 150 patients with ERCP were selected with 72 patients admitted from October 2015 to March 2017 in the control group,and 78 patients admitted from May 2017 to January 2019 in the experimental group. Patients in the control group mainly received the routine care including health education,assessment,making operation plan,and perioperative care. A project management team was established for patients in the experimental group. The team made the plans to manage the perioperative safety problems of patients in the experimental group. The effects were evaluated by comparing the complication rates,adverse event rates and nursing quality improvement in the two groups. Results The perioperative complication rate of patients in the experimental group was 5.1%(4/78),and the complication rate in the control group was 12.5%(9/72). There was no statistically significant between the two groups(χ2=2.417,P=0.120). There were 5 cases(6%)of adverse events in the experimental group and 19 cases(26%) in the control group(χ2=11.119,P=0.001). After the intervention,the care quality was improved compared with that before the intervention(P<0.05). Conclusion Safety management based on project managment is effective for ERCP patients with advanced age. It can improve the quality of nursing.

    Patients’ experience of participating in nursing bedside handover and their expectations:a qualitative study

    Objective To understand patients’ real experience of participating in nursing bedside handover and their expectations of participating in nursing bedside handover. Methods A purposive sampling methods was used to recruit 16 patients from thyroid and breast surgery department,gastrointestinal surgery department,and general internal medicine department. The semi-structured interview regarding their experience of participating in nursing bedside handover and their expectations of participating in nursing bedside handover was conducted. Content analysis was used to analyze the data. Results Three themes emerged:positive experiences(it is conducive to obtain information about one’s own diseases,is conducive to improving the relationship with nurses,and is conducive to obtaining safe care);negative experience(it is difficult to integrate into the“nursing bedside handover” and worry about the exposure of privacy and sensitive information);patient’s expectation of bedside handover(hope nurses have a positive and enthusiastic professional attitude, use easy-to-understand language, and understand patients’ preferences). Conclusion Patients have negative and positive experience of participating in bedside shift handover. It is necessary to pay attention to patients’ negative experience,and develop an individualized nursing bedside handover plan.

    Evidence summary for prevention and management of venous thromboembolism in pregnant women

    Objective To summary the best evidences for prevention and management of venous thromboembolism in pregnant women. Methods We searched the literatures on the prevention and management of maternal venous thromboembolism in the following databases:UpToDate,Cochrane Library,JBI,PubMed,CNKI,Wanfang Database,American College of Obstetricians and Gynecologists,National Institute for Health and Care Excellence,the Royal College of Obstetricians and Gynaecologists,etc. The search time limit was from database inception to December 2019.Two researchers used AGREE Ⅱ and the standards of the Australian JBI Evidence-based Health Care Center to evaluate the quality of the literature and conduct data extraction. Results A total of fourteen references were included,and they were seven clinical practice guidelines,two systematic reviews,three expert consensuses,one evidence summary,and one best practice information sheet. Twenty-five pieces of best evidence were obtained,including risk assessment,prevention measures,management strategy and health education. Conclusion This study summarizes the best evidences for the prevention and management of venous thromboembolism in pregnant women,and provides evidence for clinical managers to ensure maternal safety.

    This study introduced the structure of the rectal powder injection device. A total of 100 patients with mixed hemorrhoids in our hospital were recruited randomly from January to October 2019. These patients were divided into the experimental group(n=50) and the control group(n=50). Patients in the experimental group received the medication through the new designed device. Patients in the control group received the medication through smearing. The study showed that the new device could administer the powder to patients,save the cost of changing dressings,and reduce the number of days in hospital(P<0.05). The device is worthy of clinical promotion.

    This article summarized the current situation of interdisciplinary nursing talent training outside China from interdisciplinary curriculum,collaborative practice,research and degree. In view of the existing problems of interdisciplinary nursing talent training in China,it is suggested to build multi-dimensional and multi-directional interdisciplinary platforms,build interdisciplinary teams of teachers,create excellent interdisciplinary curriculum resources,and build a variety of interdisciplinary talent cultivation mode to promote the development of interdisciplinary nursing talent cultivation.

    This article summarized the status of Family Integrated Care research and NICU nurses’ cognition and attitude towards Family Integrated Care. We proposed several suggestions to provide a basis for further application of clinical Family Integrated Care including strengthening nursing training and education,developing professional talents and effectively using human resources to strengthen disciplinary cooperation;and exploring evaluation tools to assess nurses’ perceptions and attitudes.

    This article provided the certification information of Wound,Ostomy and Continence Nurses (WOCN) in America. It mainly focused on the agencies of the certification,specialty direction,training contents,application conditions,certification examination,and certification processes. We expect to provide suggestions for the certification of WOCN in China from the aspects of unifying and standardizing the certification institutions,constructing a feasible training system for specialty nurses and a long-term mechanism for recertification by combining the information and experience with our national practice.
