    Development of an evaluation index system for clinical thinking ability in nursing interns

    Objective To develop an evaluation index system for clinical thinking ability in nursing interns. Methods The draft index system of clinical thinking ability evaluation was formed through literature review and group meeting. Twenty-six nursing experts were consulted by emails using Delphi technique. Results The effective recovery rate of the two rounds of expert consultation was 100% and 88%,and the expert authority coefficient was 0.929 and 0.941,respectively. The Kendall coefficient of expert opinion in the second round of expert consultation was 0.293. The evaluation index system of clinical thinking ability of nursing undergraduates was constructed,which included five first-level indicators(critical thinking ability,systematic thinking ability,evidence-based thinking ability,predictive thinking ability,and innovative thinking ability) and 35 secondary indicators. Conclusion The consultation experts are positive and authoritative,and the content of evaluation index is reasonable,which can provide a basis for the evaluation of clinical thinking ability of nursing undergraduates.

    Objective To explore the construction method and teaching effect of Hospice Nursing course for nursing undergraduates. Methods Based on surveys and expert consensus,the specialized and optional course of Hospice Nursing was set up and improved. The teaching objective of the course was to guide students to establish correct attitude to life and death,master the knowledge and skills of hospice nursing,and improve their attitudes and abilities in hospice care. The teaching contents included death education,quality of life evaluation,common symptom management,psychological care of dying patients and their families,etc. Diverse teaching methods were adopted,such as case teaching,scenario simulation teaching,debate and etc. Formative evaluation was used for assessment. A total of 35 nursing undergraduates of 2018 Class were selected as the participants,and the teaching effect was evaluated by the course scores,the interview of students’ teaching satisfaction,and the surveys of students’ attitude toward death anxiety and hospice care. Results The course scores of 35 students were 87.43±0.84. In interview,18 students were satisfied with the content,method,assessment and experience of teaching,and put forward suggestions to increase teaching hours and carry out clinical practice. After taking the course,the total score of death anxiety among students was lower(Z=3.248,P<0.001),and the total score of hospice attitude was higher(t=5.284,P<0.001)than before. Conclusion These students passed the Hospice Nursing course examination smoothly and were satisfied with the teaching effect. Besides,their death anxiety got reduced with improved attitude toward hospice care.

    Objective To explore the effects of a humanistic teaching based on ICARE concept in nursing interns of a higher vocational college. Methods From October 2020 to November 2021,the nursing interns were divided into the control group(n=30) and the experimental group(n=32) according to the time they entered the clinical practice in a digestire surgery department. Students in the control group received routine teaching,while students in the experimental group received humanistic teaching based on ICARE concept. The teaching used in the experimental group including various teaching methods with the clinical cases to illustrate the humanistic spirit of integrity,compassion,accountability,respect,and excellence. At the end of practice,the two groups were compared on their humanistic care ability and caring behavior. Results At the end of 4 weeks’ practice,the scores of humanistic caring ability(190.41±5.92) and caring behavior(111.19±5.64) in the experimental group were higher than those in the control group(182.17±6.75 and 101.50±4.48,respectively)(all P<0.05). Conclusion Humanistic teaching based on ICARE concept is helpful to enhance the humanistic caring ability and stimulate caring behavior of nursing interns in the higher vocational colleges.

    By taking two nursing English micro-lectures of humanities Better Communication Better Care and Public Health and Social Care for example,the paper analyzes specifically the whole process of teaching design such as topic selection,script composition etc.,and then presents detailed technical specifications recommended for audio-visual pre-recording and post-editing of nursing micro-lectures of humanities. It proposes some suggestions and implications for designing and producing nursing micro-lectures of humanities in topic selection,script quality as well as standardized video specifications,in the hope of creating more high-quality digital nursing teaching resources.

    The present study aims to strengthen nursing students’ humanistic care ability training for the ideological and political education,and to rebuild of surgical nursing course education. The study has effectively explored the ideological and political elements and teaching strategies to tap the knowledge points of each chapter of this course from three aspects:clear curriculum education target,refine ideological elements,and design the teaching strategies and implementation. Such attempt demonstrated to be helpful to improve the humanistic quality of nursing students and cultivate nursing professionals while receiving the training of surgical nursing professional knowledge and skills.

    Objective To explore the application and effects of ideological and political teaching design in Community Nursing course focusing on professional guidance and innovative thinking training. Methods A total of 68 nursing undergraduates were selected as participants in a university. In the teaching of Community Nursing course,the ideological and political education with professional guidance and innovative thinking training as the core was carried out,which mainly included political identity,national identity,moral cultivation,and professional quality. The professional identity,self-regulated learning ability and innovation ability scales were applied for assessment before and after the intervention. Results After the course,the total scores of student’s professional identity and self-regulated learning ability were higher than those before the course(P<0.05). Also,the total score and each dimension score of the innovation ability scale were significantly higher than before(P<0.05). Conclusion The ideological and political education with professional guidance and innovative thinking training as the core can effectively improve professional identity,self-regulated learning ability and innovation ability of nursing undergraduates.

    Objective To explore the effects of the blended teaching in Thinking and Communication course on the cultural competence of nursing undergraduates. Methods From October 2021 to January 2022,two classes were assigned to the experimental group(n=169) and the control group(n=168) by tossing a coin. The blended teaching that combined online and traditional classrooms teaching was applied in the experimental group,while the routine teaching method was applied in the control group. The Chinese version Inventory for Assessing the Process of Cultural Competence among Health care Professionals(IAPCC-CV) and theoretical examination were used to evaluate the effects. Results Before the course,the cultural competence scores of the experimental group and the control group were(65.89±5.23) and(65.65±5.30) respectively,and the difference was not statistically significant(t=0.427,P=0.670). After the course,the cultural ability scores of the experimental group and the control group were(71.91±8.49) and(68.73±7.71) respectively(t=3.597,P<0.001). The theoretical scores of the experimental group(91.64±5.21)were higher than those of the control group(88.44±7.85)(t=4.407,P<0.001). Conclusion The blended teaching in Thinking and Communication course can improve the teaching effects and strengthen the students’ mastery of knowledge.

    Objective To explore the effect of curriculum design and teaching practice of Nursing Clinical Thinking Training for Common Diseases in Chinese and Western Medicine. Methods The elective course of Nursing Comprehensive Skills Training was changed to the compulsory course of Nursing Clinical Thinking Training for Common Diseases in Chinese and Western Medicine. The course content included six modules of clinical diseases(angina pectoris,cerebral hemorrhage,viral hepatitis,diabetes,herpes zoster and COPD) that could reflect the characteristics of integrated Chinese and western medicine,with seven class hours for each module,including three class hours for theory and four class hours for experiment. In addition,three class hours for design experiment and three class hours for experimental assessment. In the disease module,theoretical courses were taught by teachers of traditional Chinese medicine,clinical medicine,basic medicine and nursing. The experimental course employed the progressive standardized teaching case as the main line and carried out teaching in the form of OSCE. A total of 283 third year nursing undergraduates from a Chinese Medicine University in Hunan Province were selected as the participants in implementation. Before and after the teaching,the self-designed Chinese and western medicine nursing thinking ability scale was applied. Results The examination scores of the students were ranged from 65 to 93(81.66 ± 4.62). Besides,after such teaching,students’ thinking ability of traditional Chinese and western medicine nursing was higher than before(P<0.05). Conclusion The course of Nursing Clinical Thinking Training for Common Diseases in Chinese and Western Medicine improves the students’ clinical thinking ability of integrated Chinese and western medicine nursing.

    The current status and associated factors of work readiness in nursing graduates

    Objective To identify the current status and associated factors of graduate nurses’ work readiness. Methods We recruited 628 nursing graduates by convenience sampling. A demographic questionnaire and the Chinese version of Work Readiness Scale for Graduate Nurses were utilized to collect data. Results The average work readiness score of graduated nurses was 262.20±44.97. Multiple linear regression analysis confirmed 6 significant associated factors,including gender,education background,campus learning atmosphere,student cadre experience,voluntary choice of nursing profession,and changes in nurses’ professional identity,explaining 20.0% of the total variance. Conclusion The work readiness of graduate nurses needs to be strengthened,especially in the dimension of personal work characteristics. Nursing educators and hospital managers should pay attention to these associated factors to cultivate and recruit graduate nurses who have potentials for better career development.

    Objective To explore the mediating effect of clinical belongingness on the perceived social support and career decision-making self-efficacy. Methods This study employed a cross-sectional study design. From January to October 2021,a total of 632 nursing interns from four general hospitals in Jiangsu Province were selected by convenience sampling method. General data questionnaire,the Multidimentional Scale of Perceived Social Support,Belongingness Scale-Clinical Placement Experience,and Career Decision-making Self-efficacy Scale were used to investigate. Results Mediating effect analysis showed that clinical belongingness of nursing interns had a partial mediating effect on the relationship between perceived social support and career decision-making self-efficacy(0.166),accounting for 43.6% of the total effect. Conclusion Clinical belongingness of nursing interns plays a partially mediating role in the relationship between perceived social support and career decision-making self-efficacy. College,family and clinical medical staff can provide more emotional support,learning and work support for nursing interns to enhance their clinical belongingness,improve their career decision-making self-efficacy and promote the construction of nursing team.

    Objective To explore the effects of the multidisciplinary teamwork on skill training of prone positioning ventilation therapy in ICU. Methods A total of 64 clinical staffs were recruited in ICU of a tertiary hospital in Zhejiang Province from October 2021 to November 2021. The team strategies and tools to enhance performance and patient safety (TeamSTEPPS) was used to guide the training. The training contents included setting up a training group and training the leadership of the group leader;revising operational tools and strengthening team cooperation;training theoretical knowledge and communication tools;and setting up simulated situations and conducting experiential group exercises. After the training,the skill assessment of the prone position team was carried out,and the training experience of the participants was investigated. At the same time,fifteen people were invited for an interview. Results The score was(61.16±1.47) for skill performance,(24.38±0.97) for team cooperation,and(85.68±1.95) for the total score. All the participants believed that multidisciplinary team cooperation was helpful to improve the efficiency of implementation of prone positioning ventilation therapy,and they thought such a training could improve the cohesion of the team. All interviewees held positive attitudes towards the team training for the skill of prone positioning ventilation therapy. Conclusion The multidisciplinary teamwork model is well applied in the training of ICU prone position ventilation treatment skills,and the trainees have a positive attitude towards this training.

    Objective To investigate the current implementation status of syndrome differentiation and care in department of Traditional Chinese Medicine of 61 tertiary general hospitals in Shandong Province. Methods A total of 121 nurses recruited by cluster sampling from 61 tertiary general hospitals of Shandong Province were investigated by self-designed questionnaire method from Feb to Apr 2021. Results TCM nurses were evaluated as low for syndrome differentiation and medium for nursing care. Factors affecting the syndrome differentiation and care included being unclear syndrome differentiation,inability to fully grasp all the contents of care based on syndrome differentiation,being obscure and difficult to remember nursing contents,lacking of daily assessment requirements in functional departments,and insufficient nurse manpower. Conclusion In 61 general hospitals in Shandong Province,the nurses in the departments of Traditional Chinese Medicine are in poor condition of syndrome differentiation,and care needs to be strengthened. On-the-job training should be strengthened in nurses and a perfect guarantee system should be built up to promote the implementation of syndrome differentiation and care in traditional Chinese medical in general hospitals in depth.

    The effect of toileting behaviors on risk of urinary incontinence among the young women without pregnancy

    Objective To explore the effects of toileting behaviors on risk of urinary incontinence in a community sample of young women without pregnancy. Methods This study was conducted among 331 young women without pregnancy living in 6 communities in Xuzhou from September to December,2020. The general information questionnaire,the Chinese Version of Women’s Toileting Behavior Scale,and the International Consultation on Incontinence Questionnaire-short form were used to collect the corresponding data. Multivariate logistic regression analysis was used to investigate the relationship between toileting behaviors and urinary incontinence. Results The overall prevalence rate of urinary incontinence was 14.2% in the study. The mean score for Women’s Toileting Behavior Scale was 2.69±0.72. Premature voiding behaviors dimension was significantly associated with urinary incontinence(OR=1.151,95%CI:1.038~1.277,P=0.008). Conclusion Premature voiding behaviors increase the risk of urinary incontinence in young women. Community health care workers should carry out urological health education for young women and actively advocate correct toileting behaviors.

    Construction and application the discharge planning based on Theory of Goal Attainment in elderly patients with ostomy

    Objective To construct discharge planning for elderly patients with ostomy based on theory of goal attainment and verify the effectiveness. Methods Based on the “Theory of Goal Attainment”,we combined with the best clinical evidence and patient’s needs,and constructed the discharge planning for elderly patients with colostomy. Thirty-nine patients from April to June 2021 were in the control group and received routine care. Another 38 patients from July to October 2021 were in the experimental group,which discharge planning based on Theory of Goal Attainment was adopted. All of them come from the gastrointestinal surgery department of a hospital in Zhengzhou. After intervention,the discharge readiness,patient engagement degree of the two groups were compared,and the incidence of ostomy-related complications at discharge,the 4th and 12th week after surgery were compared between the two groups. Results At discharge,the discharge readiness score,health care participation score in the experimental group were higher than control group(P<0.05). The incidence of ostomy-related complications in the experimental group was lower than that in the control group at the 4th week after surgery(P<0.05),but there was no significant difference in the incidence of ostomy-related complications between the two groups at discharge and the 12th week after surgery(P=0.615,P=0.313). Conclusion Discharge planning based on the theory of goal attainment can improve the level of discharge readiness and patients’ participation,reduce the incidence of postoperative complications at the earlier stage after discharge.

    Nursing service quality and its influencing factors perceived by older patients in outpatient department

    Objective To understand the status and influencing factors of outpatient nursing service quality perceived by older patients. Methods A total of 619 older outpatients were investigated by the nursing service quality evaluation scale and the general information sheet. Results The total score of nursing service quality was (77.93±6.35),and the score index was 77.93%;the score of tangible was(20.69±2.32),and the score index was 82.76%;the score of assurance was(24.06±2.93),and the score index was 80.20%;the score of responsiveness was (15.12±2.33),and the score index was 75.60%;and the score of empathy was(18.05±3.48),and the score index was 72.20%. Gender,education,marital status,and the number of chronic diseases were the influencing factors of the total score of care services quality in older outpatients,which can explained 27.6% of the total variation. Conclusion The nursing care service quality perceived by olde pateints reached the qualified level. On the basis of ensuring the tangible and responsive quality of nursing services for older patients,outpatient nursing administrators should give priority to the quality of nursing services in empathy and responsiveness,and give targeted nursing intervention according to the results of influencing factor analysis,so as to achieve the continuous improvement of outpatient nursing services and improve the satisfaction of older patients.

    Objective To retrieve the evidence of wandering for patients with cognitive impairment,and to summarize the best evidence. Methods A comprehensive search was conducted in BMJ Best Practice,UpToDate,National Institute for Health and Care Research,Guideline International Network,Scottish Intercollegiate Guidelines Network,Registered Nurses Association of Ontario,National Guideline Clearinghouse,Canadian Medical Association CPG Infobase,New Zealand Guidelines Group,Australian Clinical Practice Guidelines,Association of the Scientific Medical Societies in Germany,Cochrane Library,Embase,PubMed,CNKI,Wanfang Database,Yimaitong from inception to October 15,2021. Two researchers independently evaluated the articles in accordance with the literature evaluation standards and evidence grading system of the Joanna Briggs Institute of Evidence-based Health Care in Australia,and extracted the evidence. Results A total of 12 articles were included and 49 pieces of evidence related to the management of cognitive impairment patients were compiled,including the seven aspects:risk assessment,environmental modification,behavior intervention,training and education,physical and psychological intervention,smart devices,and search strategy. Conclusion The study summarizes the best evidence related to the management of cognitive impairment patients,which can provide evidence for the development of clinical care.

    The effects of sarcopenia on the prognosis of patients with heart failure:a systematic review

    Objective To assess the effects of sarcopenia on the prognosis of patients with heart failure(HF). Methods A comprehensive search was conducted in PubMed,Web of Science,Embase,The Cochrane Library,CBM,CINAHL,CNKI,Wanfang and VIP databases from their inception to June 2022. Cohort studies related to sarcopenia and HF prognosis were collected by “snowball” method. Two reviewers independently screened articles,extracted data,and evaluated risk of bias of the included studies. Meta-analysis was then performed using RevMan 5.4 software. Results Finally 8 cohort studies involving 2 261 patients were included. The meta-analysis showed that HF patients with sarcopenia had a higher risk of all-cause death than those without sarcopenia[HR=1.32, 95%CI(1.18, 1.47),P<0.001],and sarcopenia also increases the risk of cardiogenic adverse events in HF patients[HR=1.01, 95%CI(1.01, 1.02),P<0.001]. Subgroup analysis showed that sarcopenia had no effect on all-cause death in patients with ADHF[HR=1.26, 95%CI(1.00, 1.59),P=0.05],but increased the risk of all-cause death in patients with CHF [HR=1.61,95%CI(1.09, 2.36),P=0.02]. Conclusion The results of this study indicate that sarcopenia has predictive value for the prognosis of HF patients. Due to limited quantity and quality of included studies,more high quality studies are needed to verify above conclusions.

    Objective To describe and analyze the current research status of illness perception of patients with oral diseases. Methods PubMed,Scopus,Web of Science,Cochrane Library,CNKI,VIP,WanFang,SinoMed,and OpenGrey were searched from their inception to October 2021 and updated in March 2022. The included studies were summarized and analyzed. Results A total of 14 articles were included. Three observational studies reported on the impact of illness perception on treatment adherence and outcome,three cross-sectional studies addressed factors influencing illness perception,six articles reported on assessment tools related to illness perception(one of them also reported on the influencing factors),and three reported on an interventional study related to illness perception. Conclusion Currently,few researchers have focused on the role of illness perception in promoting individual health outcomes in patients with oral diseases,and future relevant studies could be conducted in China.
