    Establishment of a training system for wound ostomy continence specialty nurses

    Objective To develop a scientific and practical training system for wound ostomy continence nurses. Methods Through extensive review of domestic and foreign literature and semi-structured interviews,a preliminary consultation questionnaire for training system of wound ostomy continence specialty nurses was developed. Delphi expert consultation method was used to form the final index system of training for wound ostomy continence specialty nurses with experts from areas of nursing management,wound ostomy continence nursing,and nursing education. Results The authoritative coefficient of experts was 0.93 and the coordination coefficient of experts was 0.479. The training system of wound ostomy continence specialty nurses included four parts. There were 6 first-level indicators and 32 second-level indicators in the training objective part. There were 5 first-level indicators,15 second-level indicators and 70 third-level indicators in the training process part. There were 4 first-level indicators and 7 second-level indicators in the training evaluation part. The qualification certification included 2 first-level indicators and 2 second-level indicators. Conclusion The reliability and practicability of wound ostomy continence specialty nurses training system are acceptable,which has a good guiding significance to carry out the training of specialty nurses.

    Objective To explore the influence of role modeling of clinical tutors on nursing undergraduates’ professional identity. Methods Twelve nursing undergraduates were interviewed by a semi-structured questions and an interpretative phenomenological analysis was carried out. Results Four themes emerged from the data including “My initial opinions on nursing”,“What I got from role modeling of clinical tutors”,“What professional identity means to me” and “How role modeling plays a role in professional identity”. These themes showed the influence of role modeling of clinical tutors on nursing undergraduates’ professional identity dynamically. Conclusion Clinical tutors’ role modeling facilitates students to see more possibilities in nursing,thus they can generate motivation and goals to improve their professional identity. Professional identity is the result of interaction between students’ inner pursuit of value and external considerations of reality,and it will be enhanced when their potential is stimulated. Students’ professional identity can be enhanced based on the clinical preceptorship which can help to gradually establish a clear career training and development path.

    Objective To evaluate the effects of a standardized debriefing applied in high fidelity simulation teaching. Methods Fifty-four nursing undergraduates of Grade 2015 were selected and divided into eight groups. All students received 40 class hour high fidelity simulation teaching from March 2018 to January 2019. The standardized debriefing(experience expression,objectives review,actions analysis,summary & reflection)was used. A self-designed questionnaire was used to evaluate the effects of the standardized debriefing. Results The standardized debriefing could make the debriefing closely target the teaching objectives and avoid digressing(100%),promote students’ self-reflection(100%),enable learners to actively participate in the discussion(100%),help the participants to identify their own shortcomings(100%),cultivate clinical reasoning ability(96.3%),promote observers to observe the "patients" carefully(96.3%),and realize the educational concept of student-centered educational learning(96.3%). Conclusion The standardized debriefing can encourage students to actively participate in debriefing,promote participants to actively reflect on their own performance,and greatly improve the effects of debriefing. Standardized defriefing is worth of reference and promotion.

    The relationship between social support and career decision-making self-efficacy of nursing students in a vocational college

    Objective To know the current status of social support and career decision-making self-efficacy of nursing students in a vocational college,and to explore the relationship between social support on career decision-making self-efficacy. Methods A total of 340 nursing students in Huangshan Vocational and Technical College were investigated with the Social Support Rating Scale and the Career Decision Making Self-efficacy Scale. Results The total score of social support was(37.94±5.77),and the total score of career decision-making self-efficacy was(115.81±28.13). There was a significant positive correlation between social support and career decision making self-efficacy(r=0.169~0.431,P<0.005). The objective support and subjective support could explain 15.3% total variation of career decision-making self-efficacy. Conclusion Social support can positively predict the career decision-making self-efficacy of nursing students in the vocational college. Nursing educators and managers should take corresponding measures to improve the level of nursing students’ career decision-making self-efficacy and enhance their career decision-making ability.

    Objective To evaluate the effects of the blended learning applied in Medical Nursing in baccalaureate nursing program. Methods The case-based online and offline teaching resources and the blended learning model in Medical Nursing was firstly constructed. Then the blended learning model was used in 151 nursing undergraduates and the effects were evaluated among this group of students. Results The average score of the online quiz was(79.36±7.38),and the score of the course academic performance was(80.95±6.19). The whole time length of teaching video on the online learning platform was about 1 023 minutes and the average watching time was(2 455.2±872.3) minutes. A total of 4 591 online discussions were conducted by 151 students. Conclusion The blended learning in Medical Nursing has obtained good teaching effects and is accpeted by the students.

    Objective To explore the effects of General Training of Midwifery Skills for nursing undergraduates. Methods The General Training of Midwifery Skills with 44 hours was designed and developed according to the clinical practice. The course was taught to 30 Year 3 students through several kinds of teaching methods:class teaching,video demonstration,group discussion,and simulation. The students were assessed through Objective Structured Clinical Examination(OSCE). Results The OSCE average scores of students was 92.21±2.01. Students were highly satisfied with the course and the teaching method. Students agreed that the course could improve their comprehensive abilities of midwifery skills. Conclusion The General Training of Midwifery Skills can improve students’ clinical skills ability,communication and problem?solving ability,team cooperation ability,and narrow the gap between experimental education and clinical performance in midwifery education.

    Objective To optimize the construction of clinical nursing teaching bases so as to strengthen the effective connections between clinical teaching in hospitals and school education. Methods With abundant teaching resources and adequate funding,we optimized the clinical nursing teaching bases from the following aspects:the organizational structure,management system,teaching staff,teaching practice model,and nursing teaching quality supervision system. Clinical instructors were trained through 'inviting in experts,sending them out for learning,and strengthening training'. The qualities of students were improved based on the 'three stages' and 'five characteristics' clinical nursing teaching and training model. Besides,we established a teaching quality supervision system of mutual evaluation among teaching departments,teachers,and students. Results The teaching quality was improved continuously. The structure of the teaching staff team was optimized. The comprehensive teaching abilities were increased. The satisfaction level of students with the teaching was increased year by year. Conclusion Optimizing the construction of clinical nursing teaching bases is an effective way to help with the hospital teaching reform and strengthen the close connection between clinical teaching and school education.

    The design and application of a neurology clinical teaching for higher vocational nursing students

    Objective To explore the effects of a blended teaching in clinical teaching of Neurology for higher vocational nursing students. Methods The students in the Neurology Department were divided into the control group and the interventional group. Offline teaching was used in the control group while the “online + offline” blended teaching was used in the interventional group. Offline teaching covered one-to-one clinical teaching and class teaching. Online teaching was carried out through online teaching platform. The teaching assignments,learning,and assessment were all released through the platform. Students could learn the related knowledge through the platform. Instructors could check the learning results during clinical practice. The examination results and teaching satisfaction of the students in the two groups were compared and evaluated. Results The mean examination score of the interventional group was 88.23±2.60 which was significantly higher than that of the control group(83.96±4.58)(P<0.01). The learning effects and satisfaction of students in the interventional group were significantly higher than those of the control group(P<0.01). Conclusion The blended teaching can effectively improve the quality of clinical teaching and satisfaction of nursing students in the Neurology Department.

    Objective To evaluate the effects of a core-competency training program in community nurses. Methods Sixty-four community nurses who participated in Community Specialist Nurse Training received a core-competency training for four weeks in Chongqing. The outcomes were evaluated by conducting examination and distributing the Competency Inventory for Community Nurse questionnaire before and after training. Nurses’ satisfaction level was assessed after the training. Semi-structured interviews with nurses,managers and colleagues were conducted half a year after the training. Results After the training,the examination performance and core competency scores were higher than those of baseline(P<0.05). Besides,nurses had a high satisfaction degree with the training. All participants who participated in the interviews believed that they had improved competence in clinical practice,interpersonal communication,clinical critical thinking,as well as research and teaching after the training. Conclusion The core-competency training is worthy of promotion for it improves multiple core competences in community nurses.

    Objective To develop a scientific and standardized training system for cardiovascular nurses. Methods The training objectives and system framework were determined by literature analysis and expert consultation meeting. Delphi method was used to carry out two rounds of expert consultation,and five parts including training objectives,requirements for instructors,training contents and hours,training methods and assessment were established. Results The effective response rates for both two rounds of expert consultations were 95%. The expert authority coefficient was 0.88,and the coefficients varied from 0 to 0.232. The training objective for cardiovascular specialty nurses was to cultivate the characteristic specialized nurses. The training team should include experts and instructors. The 1-month theoretical training included four parts:basic theoretical knowledge,professional skills,relevant professional abilities,and professional characteristics. Class teaching and skill training should be used to conduct the theoretical training. The 2-month clinical practice was carried out by clinical teaching and online learning. The assessment included theoretical assessment,clinical skill assessment,and training summary and case analysis assessment. Conclusion The training system of cardiovascular specialty nurses is scientific and feasible,which can provide reference for clinical cardiovascular specialty nurses training.

    Objective To explore the effect of anesthesia nurse training in Nanjing. Methods Twenty two nurses of anesthesiology department in Nanjing were trained,including one week of public basic courses(40 class hours),four weeks of professional courses(176 class hours),and seven weeks of clinical practice. Finally,the evaluation results of nurses and the satisfaction level of training were evaluated. Results After training,the scores of theoretical assessment of nurses were higher than that before training,the difference was statistically significant(t=2.690,P=0.012),the operation and practice assessment were up to the standard level(>80 points),and the overall satisfaction of nurses for training was 94.8%. Conclusion The training of anesthesiologists in Nanjing can effectively improve the knowledge and skills of anesthesiologists,and lay a foundation for establishing a scientific and systematic training model of anesthesiologists,which is worth of promotion.

    The application of the virtual reality technology with intelligence teaching tools in imaging training of neurosurgical nurses

    Objective To explore the effects of virtual reality(VR) with intelligence teaching tools in imaging training of neurosurgical nurses. Methods A total of 60 nurses were selected by convenience sampling. A 8-week imaging training program with VR and intelligence teaching tools was carried out. Then a 14-week imageology interpretation was conducted. The training effects were evaluated through theory examination and actual imageology interpretation. Besides,nurses was investigated by the neurosurgical imaging core competence questionnaire and nurse satisfaction questionnaire. Results The mean score of theory examination was 91.26±3.13. After the training,the nurses’ ability of imageology interpretation and score of neurosurgical imaging core competence were higher than those before the training(P<0.05). The total satisfaction score with the VR and intelligence teaching tools was 18.52±1.36. Conclusion The VR with intelligence teaching tools can consolidate the teaching quality and improve the accuracy level of imageology interpretation. The training method has been accepted by nurses and is worthy of further promotion.

    Objective To explore the effects of a systematic training of airway management ability of the junior nurses in the Intentive Care Unit(ICU). Methods The airway management training group was set up to systematically train 54 junior ICU nurses. The theory teaching was combined with online teaching and offline salons. The bedside teaching,guidance and situation simulation were also used. Cases of adverse events related to airway management were analyzed. The airway management ability was compared before and after the intervention. Results After the training,the scores of sucking ability of junior nurses were higher than those before the intervention(t=3.911,P=0.003). The qualification rate of airbag management was higher than that before the intervention( χ2=66.799,P<0.001). The qualification rate of humidification management was higher than that before the intervention( χ2=10.581,P=0.001). Conclusion The systematic training has effectively improved the airway management ability of ICU junior nurses.

    Objective To investigate the status of the enhanced recovery after surgery(ERAS) in the department of Orthopaedics in hospitals in Luohe. Methods We conducted an investigation on application status of ERAS strategy in the department of orthopaedics in hospitals in Luohe. Results The items of ERAS which were not well implemented included the shortening the fasting time before anesthesia,cleaning skin before operation without shaving,unconventional utilization of tourniquet,keeping warm during operation,preemptive analgesia and multimodal analgesia,and actively preventing nausea and vomiting. Items that were implemented optimally included encouraging balloon blowing and other measures to improve cardiopulmonary function,prophylactic utilization of antibiotics before operation,prevention of thrombus after operation,not using utilization or removing the catheter 48 hours after operation,and early activity of getting out of bed. Conclusion The ERAS has not been fully implemented in the department of orthopaedics in hospitals of Luohe. Health professionals should implement it according to the characteristics of local patients so as to achieve the goals of promoting patients’ rehabilitation and improving medical efficiency.

    Objective To explore the effects of a pressure injury management strategy by the wound ostomy specialty nurses. Methods We firstly analyzed the pressure injury management needs in our hospital. Then the management strategy of pressure injury which was led by the wound ostomy specialty nurses was developed and constructed. These strategies included personnel selection,team construction,making good use of the functions of specialty nurses,management process standardization,and establishing the monitoring system of pressure injury. Results There was a significant difference in qualification rate of the theory and skill assessments regarding the pressure injury management(P<0.01). The incidence of stress injury in our hospital decreased year by year from 2016 to 2018(P<0.01). Conclusion The pressure injury management model can make the wound ostomy specialty nurses play their roles completely. The model can improve the abilities of wound ostomy specialty nurses,decrease the incidence of pressure injury,relieve the sufferings of patients,and increase the social benefits of the hospital.

    Objective To explore the current status of optimism and quality of life in systemic lupus erythematosus(SLE) patients,and analyze the correlation between the two factors. Methods Totally 126 SLE hospitalized patients in a tertiary hospital in Zhengzhou were investigated by general information questionnaire,the Life Orientation Test(LOT) and Medical Outcomes Study Short Form 36(SF-36). The correlation between the two factors was analyzed. Results The score of LOT in SLE patients was(13.82±2.81). The SF-36 score in SLE patients was66.8(53.4,89.3). There were significant differences in optimism score in SLE patients with different education background and SLE disease activity index(P<0.05). The LOT score in SLE patients was positively correlated with the SF-36 score. Conclusion The optimism and quality of life in SLE patients were on a low level. Nurses should pay attention to patients’ physical and mental state,help patients to establish positive and healthy thinking and behavior patterns,and improve patients’ optimistic psychological level and quality of life.
