机器人辅助手术护理配合模块化培训方案的构建及实证研究

Construction and effect analysis of a modular training program for robot-assisted surgical nursing coordination

摘要:

目的 构建机器人辅助手术(robotic-assisted surgery,RAS)护理配合模块化培训方案,并验证其效果。 方法 采用文献分析、半结构访谈、德尔菲、小组讨论法,形成RAS护理配合模块化培训方案。选取某院2022年1月—2022年12月接受RAS护理配合培训的21名手术室护士作为对照组,对其实施常规培训;选取2023年1月—2023年12月该院接受RAS护理配合培训的21名手术室护士作为试验组,对其实施基于RAS护理配合模块化培训方案的培训。比较两组理论知识及操作技能考核成绩、护理配合项目完成时间、医生满意度得分。结果 试验组理论知识及操作技能考核成绩、护理配合项目完成时间(包括术前用物准备时间、机械臂保护套安装时间、RAS系统3D镜头校正时间、移动床旁机械臂至指定位置时间、术中故障识别及处理时间、术后器械分类整理时间)、医生满意度得分均优于对照组(P<0.05)。 结论 本研究构建的RAS护理配合模块化培训方案具有一定的科学性和实用性,提高了手术室护士RAS护理配合相关知识和技能水平,缩短了RAS护理配合项目完成时间,提升了医生的满意度,有助于保障RAS质量安全。

Abstract:

Objective To construct a modular training program for robotic-assisted surgery (RAS) nursing coordination and explore its effects. Methods A modular training program for RAS nursing coordination was developed through literature analysis,semi-structured interviews,group discussions,and Delphi method. A control group of 21 operating room nurses who received RAS nursing coordination training from January 2022 to December 2022 was selected and underwent conventional training. Another experimental group of 21 operating room nurses who received RAS nursing coordination training from January to December 2023 was selected and underwent training based on the modular training program for RAS nursing coordination. The theoretical knowledge and operational skills assessment scores,completion time of nursing coordination items,and surgeon satisfaction scores were collected and compared between the two groups. Results The experimental group showed better performance in theoretical knowledge and operational skills assessment scores,less completion time of nursing coordination items (including preoperative material preparation time,robotic arm protective sleeve installation time,robot 3D lens calibration time,time to move the bedside robotic arm to the designated position,intraoperative fault identification and handling time,and postoperative equipment sorting time),and higher surgeon satisfaction scores compared to the control group(P<0.05). Conclusion The modular training program for RAS nursing coordination constructed in this study demonstrates scientific rigor and practicality,improving the knowledge and skill of operating room nurses related to RAS nursing coordination,shortening completion time of nursing coordination items,enhancing the satisfaction of RAS surgeons,therefore,contributing to the safety and quality of RAS surgeries.
