The effects of health education based on problem classification for postoperative gastric cancer patients with home nutrition support

摘要:

目的 探讨以问题分类为基础的健康教育在胃癌术后家庭营养支持中的应用效果。 方法 2018年4月—2019年4月,将84例胃癌手术患者随机分为对照组(n=42)和试验组(n=42),对照组采用常规药物治疗和围术期护理,试验组在此基础上实施以问题分类为基础的健康教育,即通过问题诊断、问题探究、问题建模、问题解决进行健康教育。干预后,比较两组饮食健康知识掌握情况、营养指标、营养评分及并发症的发生率。 结果 干预后试验组饮食健康知识掌握情况优于对照组(P<0.05),试验组体重、白蛋白、前白蛋白、转铁蛋白、视黄醇结合蛋白、球蛋白、淋巴细胞指标较对照组高,营养风险筛查评分及患者主观整体营养状况评分较对照组改善,试验组并发症发生率低于对照组,各项目差异均具有统计学意义(P<0.05)。结论 以问题分类为基础的健康教育可提高胃癌患者术后饮食健康知识水平和营养指标,降低并发症发生率。

Abstract:

Objective To explore the application effects of problem-based health education on family nutrition support for patients with gastric cancer after surgery. Methods A total of 84 postoperative gastric cancer patients were randomly divided into the control group(n=42) and the interventional group(n=42). Patients in the control group received routine care while patients in the experimental group received health education based on problem classification. The problem classification included problem identification,problem summary,problem categorization,and problem solving. After the intervention,the two groups’ dietary health knowledge,nutritional indicators,nutritional scores and incidence of complications were compared. Results The score of diet knowledge in the interventional group was higher than that in control group(P<0.05). The weight,albumin,prealbumin,transferrin,retinol binding protein,globulin and lymphocyte index in interventional group were all higher than those in the control group. The Nutrition Risk Screening 2002 and Patient-Generated Subjective Global Assessment were improved,and the complication rate was lower than that in control group. All comparisons were statistically different(P<0.05). Conclusion The health education based on problem classification can significantly improve the level of dietary health knowledge and nutritional indicators of postoperative gastric cancer patients and reduce complication rate.
