    Construction of training contents of nursing informatics competency for nurses

    Objective To construct a scientific and normative training contents of nursing informatics competency for nurses. Methods A draft of the training contents was formed through literature review and focus group meeting. Fourteen nursing informatics experts from 10 provinces and cities in China were invited to conduct two rounds of consultation for final contents finalization. Results The response rate of the two rounds of Delphi was 100%. The expert authority coefficient was 0.91. The Kendall’s W of the two rounds of expert consultation was 0.210 and 0.350 respectively(P<0.001). The established training contents were composed of 3 parts:basic theoretical knowledge,professional skills and relevant professional ability. The contents included 10 first-level items such as nursing informatics knowledge,nursing information system development and professional leadership and 33 second-level items such as clinical nursing information needs collection and analysis,a multidisciplinary team cooperation model and the patent application strategy and achievements transformation. Conclusion The training contents of nursing informatics competency for nurses are scientific,which can provide reference for cultivating talents in nursing informatics field.

    Evaluation and intervention of medication compliance in type 2 diabetic patients

    Objective To explore the effects of a medication adherence program based on trans-theoretical model in patients with type 2 diabetes. Methods The convenience sampling method was uesed to recruit 136 patients with type 2 diabetes mellitus in a community from January to August 2019. Participants were assigned to the experimental group(n=70) and the control group(n=66) randomly. Based on the health cabin platform,the trans-theoretical model intervention was constructed and implemented in the experimental group. According to the self-designed stage of change questionnaire in medication compliance,behaviors of participants in the experimental group were divided into 5 stages:pre-contemplation,contemplation,preparation,action and maintenance. Corresponding interventions were taken for patients in each stage. Meanwhile,patients in the control group participated received routine services provided by the wisdom service center. The scores of medication adherence and medication self-efficacy in the two groups were compared before the intervention,6 months and 12 months after the intervention. The glucose metabolism indicators were evaluated before the intervention and 6 months after the intervention. Results There were intergroup and time interaction effects in medication adherence and medication self-efficacy(F=100.772,F=15.827,P<0.05). Simple effect analysis showed that medication adherence and medication self-efficacy scores in the experimental group were higher than those in the control group 6 months and 12 months after the intervention. In addition,the glucose metabolism indicators in the experimental group were significantly superior to those in the control group 6 months after the intervention(P<0.05). Conclusion The intervention program for medication compliance of patients with type 2 diabetes based on a trans-theoretical model can effectively improve the medication self-efficacy,medication adherence,and blood glucose control effect of type 2 diabetes patients.

    Objective To develop a health education program with “offline lecture + online supervision” and to examine the effects of the program. Methods A total of 92 patients admitted from May 2019 to May 2020 were recruited and assigned to the experimental group. The offline lectures were provided to the patients in the experimental group during their hospitalization. After discharge,the online supervision was provided to supervise the home care. Another 103 patients who were admitted to the hospital from April 2018 to April 2019 were recruited and assigned to the control group and traditional health education was given to them. The quality of discharge education and self-care ability of patients at admission,discharge,1 month after discharge,and 3 months after discharge in the two groups were compared. Besides,the incidence of stoma-related complications within 3 months after discharge was recorded. Results There was no statistically significant difference in the dimensional score of discharge knowledge need contents between the two groups(P>0.05). The knowledge acquisition dimension score(51.08±2.94),the teaching skills and effects dimension score(100.37±6.41),and the education quality total score(152.01±7.96) of patients in the experimental group were higher than those in the control group(P<0.05). There was an interaction effect between the patient’s self-care ability and time,and the simple effect analysis showed that at the times of discharge,1 month after discharge,3 months after discharge,the self-care ability scores between the two groups were statistically different(P<0.05). Complications within 3 months after discharge of patients in the experimental group(9.78%) was lower than that of the control group(63.11%)(P<0.05). Conclusion The “offline lecture + online supervision” can improve self-care ability when patients are at home,reduce the incidence of adverse events in patients with enterostomy,and ensure patient safety effectively.

    The mediating effect of coping style between perceived stress and sleep quality in young and middle-aged patients with coronary heart disease

    Objective To investigate the mediating effect of coping style on perceived stress and sleep quality in young and middle-aged patients with coronary heart disease. Methods From August 2019 to July 2020,general information questionnaire,Medical Coping Modes Questionnaire,Perceived Stress Scale and Pittsburgh Sleep Quality Index were used to investigate 242 young and middle-aged patients with coronary heart disease in 5 affiliated hospitals of Xinxiang Medical University. Results The total score of perceived stress in young and middle-aged patients with coronary heart disease was(30.71±7.39). The score of confrontation dimension of coping mode was(18.11±3.93),avoidance dimension was(16.36±2.76),and resignation dimension was(9.86±3.10). The sleep quality score was(7.44±3.60). Perceived stress and coping mode explained 46.9% of the total variation of sleep quality,and coping mode mediated the relationship between perceived stress and sleep quality(confrontation:0.155;avoidance:0.079;resignation:0.087),P<0.05. Conclusion Perceived stress and coping mode can predict sleep quality in young and middle-aged patients with coronary heart disease,and coping mode plays a mediating role between perceived stress and sleep quality. Medical staff should pay attention to the coping mode of young and middle-aged patients with coronary heart disease and evaluate the coping mode of hospitalized patients,in order to take targeted measures to reduce their stress and improve their sleep quality.

    Summary of the best evidence for nutrition management in patients with stable chronic obstructive pulmonary disease

    Objective To comprehensively search and analyze the relevant evidence of nutrition management in patients with stable chronic obstructive pulmonary disease(COPD). Methods We searched BMJ Best Practice,UpToDate,Scopus,Web of Science,SinoMed,CNKI,Wanfang,VIP,and Global Initiative For Chronic Obstructive Lung Disease from January 1,2010 to January 31,2021 to collect the literature including guidelines,expert consensuses,evidence summaries,systematic reviews,meta-analyses and randomized controlled trail(RCT). Two researchers independently evaluated the quality of the relevant literature,extracted and summarized the evidence. Results A total of 14 articles were included,including 7 guidelines,2 expert consensus,3 systematic reviews,1 RCT,and 1 evidence summary. Finally,25 pieces of best evidence were formed involving 6 aspects:organizational structure of nutrition management,nutrition risk screening,nutrition assessment,nutrition support,diet management,and weight monitoring. Conclusion This study summarized the best evidence of nutrition management for patients with stable COPD. Clinical medical staff should choose the best evidence according to the clinical situation,patients’ wishes,the facilitors and barriers of evidence application,to form a standardized,effective and individualized nutrition management schedule.

    The effects of empowerment education on rehabilitation in stroke patients:a meta-analysis

    Objective To systematically review the effects of empowerment education on rehabilitation of stroke patients. Methods A computer retrieval was conducted in Cochrane Library,PubMed,Embase,CINAHL Complete,SinoMed,CNKI,Wanfang Data and VIP Database,from their inception to July 2021. The included articles were evaluated according to the Cochrane manual 5.1.0 quality evaluation criteria,and meta-analysis was performed using RevMan 5.4 statistical software. Results A total of 21 studies involving 2 242 patients were included. The results showed that empowerment education could improve limb movement function[SMD=1.24,95%CI(1.02,1.45),P<0.001],cognitive function[SMD=2.07,95%CI(1.77,2.38),P<0.001],self-efficacy[SMD=1.32,95%CI(1.14,1.50),P<0.001],and activities of daily living[SMD=1.40,95%CI(1.02,1.79),P<0.001]. The improvement of self-management ability was also significantly better than that of the control group(P<0.05). Conclusion Empowerment education can improve the limb movement function,cognitive function,self-efficacy,self-management and self-care ability of stroke patients,but this conclusion still needs to be verified by larger sample and more randomized controlled trials with higher quality.

    The effects of exercise combined with cognitive training in people with mild cognitive impairment and Alzheimer’s disease:an integrated review

    Objective To understand the current research status of exercise combined with cognitive training(ECT)in patients with mild cognitive impairment or Alzheimer’s disease. Methods We synthesized the studies according to the integrated review framework proposed by Whittemore and Knafl. Results A total of 33 articles from 28 studies were included. This review divided the combination modes of ECT into three types:dual-task,interactive and sequential types. Different modalities had various research schemes:the dual-task mode had the longest duration and the forms of interventions were relatively simple;the interactive mode was similar with dual-task mode in frequency and the duration of each session. Most studies of interactive mode adopted aerobic exercise combined with computer games. The sequential mode had the longest weekly time and the most frequency,and the forms of exercise and cognitive training were relatively complex. In addition,the review found that 18 articles(72.0%) from 25 studies with control group were dual-arm,and 13 articles(46.4%) reported adherence rates. Effectiveness evaluation indicators included cognitive function,psycho-behavioral symptoms,activities of daily living,quality of life,gait,etc. Conclusion The intervention schemes need to be further standardized. Future studies should pay more attention to reasonable control group,adherence,and objective indicators.

    Bilateral arm training plays an important role in the rehabilitation of stroke patients. Researchers focus on the function of unaffected limb and apply the synchronous training of unaffected and affected limbs in stroke rehabilitation in various forms. This paper summarized the target subjects,practitioners,intervention period,training time,training form and training effects of bilateral upper limb training to provide reference for clinical nursing practitioners.

    Objective To translate the Academic Entitlement Questionnaire(AEQ) into Chinese and to examine its reliability and validity in Chinese university students. Methods The English version of the Academic Entitlement Questionnaire was translated,back translated and modified for cultural adaptation. The Chinese version of Academic Entitlement Questionnaire was used to investigate 1102 college students in Guizhou province. Another 121 students were followed to complete the Chinese version of AEQ four weeks after the first investigation. Results The internal consistency reliability and retest reliability of the scale were 0.76 and 0.70,respectively(P<0.01). The results of confirmatory factor analysis showed that CFI,TLI,IFI,GFI,NFI,RMSEA and SRMR were 0.93,0.90,0.93,0.97,0.91,0.07 and 0.04 respectively. The factor loading of each item ranged from 0.43 to 0.69. Conclusion The Chinese version of the AEQ has good reliability and validity and can be used to measure the academic entitlement of Chinese college students.

    The research ability of nursing undergraduates is the key to the sustainable development of nursing. The School of Nursing,Tianjin University of Traditional Chinese Medicine,cultivates students’ scientific research ability in curriculum design and scientific research practice,including scientific research enlightenment training,scientific research introductory training,scientific research exploration training,and scientific research ability integration. The second classroom is also carried out to increase students’ research interests. All these measures have good effects on students’ scientific research ability. The experience can provide references for improving the scientific research ability of nursing undergraduates.

    Objective To explore the teaching strategies and effects of ideological and political theory education in undergraduate Nursing Research. Methods The teaching reform was performed among 255 undergraduates of Year 3 within six classes. The teaching reform strategies were implemented in terms of finding wealth of ideological and political elements,integrating the elements into the knowledge system,developing rich resources for ideological and political theory education online,and optimizing teaching approaches and channels offline. The effects of the teaching reform were evaluated through students’ acceptance,and their learning experience. The peer evaluation of teaching performance was also conducted. Results Students showed high acceptance rates of the teaching strategies. Specifically,for the breadth,depth,logicality,and overall evaluation of ideological and political theory teaching,the proportions of students’ grading on“strongly agree” were 71.4%,67.0%,76.5% and 68.6%,respectively. The experience of students’ learning was also positive. They found that the elements of ideological and political theory teaching were various in content,style,and theme,and these elements were also logical and natural when blended with knowledge of research,and the teaching was found effective in fostering students’ virtue through education. In addition,the course was also awarded the first prize in micro course competition of ideological and political theory teaching in Jiangsu Province. Conclusion The ideological and political theory teaching of Nursing Research can both improve students’ ideological quality and research knowledge level,which is highly acceptable by students and peers.

    Investigation on clinical practice behaviors of male nursing students during internship

    Objective To investigate the status of male nursing students’ clinical practice behaviors during internship. Methods A total of 189 male nursing students were investigated by general data questionnaire and clinical practice behavior evaluation of nursing undergraduates in 5 tertiary general hospitals in Jinan,Shandong province. Results The total score of clinical practice behaviors was(105.00±13.69),and the score of emotion dimension,operation skill dimension and cognition dimension were(3.90±0.53),(3.83±0.57),(3.49±0.52) respectively. The three items that scored highest“protect patient privacy”“treat patients equally”and“explain to patients before performing care procedures”were all in the emotional dimension. The three items with the lowest scores“revising nursing plan”“making individual nursing goals”and“evaluating nursing effects”all belonged to the cognitive dimension. 88.9% male students liked nursing and 82.0% wanted to be a nurse after graduation. Students who liked nursing or not,attended hospital internship training or not,were satisfied with the clinical instructors or not,and were willing to be a nurse after graduation or not had different scores of clinical practice behaviors(P<0.05). Conclusion Male nursing students’ clinical practice behaviors level is above average and their cognitive performance is relatively poor. Nursing educators and administrators should provide professional and emotional education and career planning guidance to male students,conduct regular job training,and cultivate the qualified clinical instructors to promote the improvement of clinical practice ability of male students.

    The effects of “shadow” experience activities in clinical teaching in the orthopedic department

    Objective To explore the application effects of “shadow” experience activities in teaching of nursing interns in the orthopedic department. Methods A total of 102 nursing interns practicing in a tertiary hospital from August to December 2020 were recruited and divided into the control group and the experimental group randomly. Students in the control group received the routine teaching. Students in the experimental group received“shadow”experience activity including training,demonstration and perception;following,experiencing and transposition;sharing,reflection and internalization;and feedback and assessment on every week. After the teaching,students’ clinical comprehensive ability,examination performance,and skills assessment were compared between the two groups. Results The score of the Mini-Clinical Evaluation Exercise in the experimental group was(7.68±0.84),and the control group was(7.04±1.28). The difference between the two groups was statistically significant(t=2.995,P=0.004). The examination performance and skill assessment of students in the experimental group were all better than those of students in the control group(P<0.05). Conclusion The “shadow” experience activities used in the teaching of nursing interns in orthopedic department can improve the clinical comprehensive ability,examination performance and skills of nursing students.

    A survey of patient safety competency of nurses in tertiary hospitals in Shanxi Province

    Objective To investigate the status of patient safety competency of nurses in tertiary hospitals in Shanxi Province. Methods A proportional stratified cluster sampling method was used to select 2,417 nurses from 11 tertiary hospitals in Shanxi Province. The general information questionnaire and the Patient Safety Competency Nurse Evaluation Scale were used. Results The overall score of patient safety competency of nurses in tertiary hospitals was(82.28±13.05). Nurses with different departments,age,working experience,professional title,positions,and average monthly night shift were different in patient safety competency(P<0.05). Conclusion The survey sample is well representative. The patient safety competency of nurses is not influenced by gender and education. It might be influenced by their departments. Nurses with older age,higher position and professional title,longer working experience,and less monthly night shifts have higher patient safety competency.

    Objective To understand the current work pressure of new nurses in Fujian province. Methods A mixed-method study design was adopted. The Chinese Nurses Stressor Scale was used to investigate 182 new nurses in 4 comprehensive tertiary hospitals in Fuzhou. Semi-structured in-depth interviews were conducted with 12 new nurses. Results The results of questionnaire survey showed that the pressure score of new nurses was (2.56±0.31),which was in the upper middle level. The time allocation and workload dimension scores were the highest (2.95±0.46). The work pressure of new nurses with different genders,living conditions,and departments was different(P<0.05). Five themes were extracted from the interview data including workload and work responsibilities as the main sources of pressure,insufficient knowledge and unskilled operations that affect work confidence,low job accomplishment leading to maladaptation of the role of nurses,poor pressure regulation affecting physical and mental health and work status,and colleagues’ help and social support as the main resources pressure relieving. Conclusion The overall work pressure of new nurses is relatively high in the early stage of their career,and the pressure source is diverse. Nursing managers should actively pay attention to the work pressure of nurses and provide targeted interventions to help them successfully complete the role transition.

    Translation and validation of the Nursing Student’s Knowledge about Breastfeeding Questionnaire

    Objective To develop the Chinese version of Nursing Student’s Knowledge about Breastfeeding Questionnaire and examine its reliability and validity. Methods According to the Brislin’s translation model,we followed the forward-translation and back-translation procedure to translate the questionnaire into Chinese,modified it based on Chinese culture. The Chinese version was revised through experts’ review and a pilot study. The convenience sampling method was used to investigate 134 nursing students. The reliability and validity of the questionnaire were tested by exploratory factor analysis,content validity,Cronbach’s α,and retest reliability. Results The final Chinese version of the Nursing Student’s Knowledge about Breastfeeding Questionnaire is a single-dimensional questionnaire with 14 items. The content validity of the total questionnaire was 0.963,and each item was 0.710~1. Cronbach’s α coefficient of the total questionnaire was 0.763,and each item was 0.613~0.752. The split-half reliability was 0.780,and the retest reliability was 0.813. Exploratory factor analysis extracted 4 common factors,and the cumulative variance contribution rate was 55.266%. Conclusion The Chinese version of Nursing Student’s Knowledge about Breastfeeding Questionnaire was demonstrated to be reliable and valid among nursing students.
