突发重大传染病事件中护士应对伦理困境培训方案构建及应用研究

Construction and application of a training program for nurses addressing ethical dilemmas in emergent major infectious disease events

摘要:

目的 构建突发重大传染病(emergent major infectious disease,EMID)事件中护士应对伦理困境的培训方案,并对其应用的可行性和效果进行探索。 方法 通过访谈、叙事素材收集、文献研究及德尔菲法构建培训方案。于2024年9月—10月对上海市某三级甲等医院招募的55名护士实施培训,采用叙事教学法,并通过EMID事件中护士伦理困境量表、护理伦理决策问卷、应对方式问卷及满意度调查评价培训效果。 结果 培训后当日及1个月与培训前比较,护士伦理困境量表得分降低(P<0.05);护理伦理决策能力总分及伦理困境应对方式中积极应对方式维度得分均升高(均P<0.05)。培训满意度调查结果显示,10项指标满意度均高于92%,学员满意度得分为(92.00±6.50)分。结论 经过初步应用,EMID事件中护士应对伦理困境培训方案表现出良好的可行性与接受度,有助于提升护士在突发重大传染病事件中伦理决策能力,促使其积极应对伦理困境。

Abstract:

Objective To develop a training program for nurses to address ethical dilemmas during emergent major infectious disease(EMID) events and to explore its feasibility and effectiveness in application. Methods The training program was developed through qualitative interviews,collection of narrative materials,literature review,and Delphi method. From September to October 2024,55 nurses were recruited via convenience sampling from a tertiary Grade-A hospital in Shanghai to participate in the training. Training effectiveness was evaluated using the Nurse Ethical Dilemma Scale in EMID events,the Judgments About Nursing Decision,the Coping Style Questionnaire,and a satisfaction survey. Results Compared with the pre-training baseline,scores on the Ethical Dilemma Scale were significantly lower immediately after training and at the one-month follow-up(all P<0.05). Conversely,total scores for nursing ethical decision-making ability and the positive coping dimension of the Coping Style Questionnaire significantly increased(all P<0.05). The training satisfaction survey showed that satisfaction rates for all 10 indicators exceeded 92%,with a mean participant satisfaction score of 92.00±6.50. Conclusion The training program demonstrated good feasibility and high acceptability. Implementation of this program effectively enhanced nurses’ ethical decision-making abilities and promoted positive coping strategies in EMID events.
