ICU医护人员跨专业谵妄管理培训实践与效果研究

Effect of interprofessional delirium management training for ICU physician-nurse

摘要:

目的 探讨跨专业谵妄管理培训在ICU医护人员中的应用效果。 方法 于2024年8月—10月对河南省某三级甲等医院ICU的48名护士和16名医生实施跨专业谵妄管理培训,培训内容包括理论授课和跨专业模拟训练。比较医护人员培训前(T0)、培训结束时(T1)、培训后2个月(T2)的谵妄知识得分,并通过焦点小组访谈法了解培训对医护人员跨专业谵妄管理的影响,以及医护人员对培训的看法及建议。 结果 T1及T2阶段医护人员谵妄知识得分均较T0阶段有所提升,差异有统计学意义(P<0.05)。T2阶段医护人员谵妄知识得分较T1略有下降(P<0.05)。焦点小组访谈结果显示,医护人员对培训认可度高但仍需优化;培训为医护人员带来了谵妄认知及行为方面的收获与改变;谵妄知识向实践转化仍存在障碍。 结论 针对医护人员的谵妄管理培训得到了积极的评价,并且能够提高ICU医护人员对谵妄的认知,深化医护人员谵妄管理临床实践。

Abstract:

Objective To explore the application effect of interprofessional delirium management training in ICU healthcare workers. Methods Interprofessional delirium training was implemented from August 2024 to October 2024 for 48 nurses and 16 doctors in the ICU of a tertiary hospital in Henan Province,which included theoretical lectures and interprofessional simulation training. The delirium knowledge scores of the medical staff before training (T0), at the end of training(T1), and two months after training(T2) were compared. Meanwhile, the views and suggestions of the nurses and doctors on the training were understood through focus group interviews. Results The delirium knowledge scores increased immediately after training (T1) and two months after training (T2) compared to pre-training(T0), and the difference was statistically significant(P<0.05). The delirium knowledge score of medical staff at T2 was slightly lower than that at T1(P<0.05). Focus group interviews revealed that healthcare professionals recognized the training highly but still needed optimization;the training brought gains and changes in delirium cognition and behavior for healthcare professionals;and barriers to translating delirium knowledge to practice still existed. Conclusion The interprofessional delirium training was positively evaluated and was able to improve ICU healthcare professionals’ knowledge of delirium and deepen clinical practice in healthcare delirium management.
