视网膜中央动脉阻塞患者就医延迟研究进展

Review of medical delay in patients with central retinal artery occlusion

摘要:

就医延迟(包括院前延迟和院内延迟)是导致视网膜中央动脉阻塞患者溶栓率低的主要原因。该文对视网膜中央动脉阻塞患者就医延迟的概念内涵及就医延迟影响因素等进行综述。视网膜中央动脉阻塞患者就医院前延迟影响因素包括人口统计学因素、症状感知水平、认知和相关知识水平、就诊和转运模式;院内延迟影响因素包括医疗资源有限和水平不足、院间转诊未成体系、缺乏相关标准化救治流程。建议将单眼突发性无痛性视力丧失纳入院前卒中评估,提高公众对疾病的识别度;构建视网膜中央动脉阻塞绿色通道,缩短到院至溶栓时间,进而改善视网膜中央动脉阻塞患者就医延迟的状况。

Abstract:

Medical delay (including pre-hospital and in-hospital delay) is the main cause for the low thrombolytic rate in patients with central retinal artery occlusion. In this paper,we described the concept of medical delay and reviewed the influencing factors. The influencing factors of prehospital delay included demographic factors,symptom perception level,awareness and related knowledge level,visit and transport mode. Through reviewing,limited and inadequate medical resources,incomplete inter-hospital referral system,and lack of standardized treatment procedures were found to be the factors affecting in-hospital delay. After analysis,this paper suggested that sudden painless vision loss in one eye should be included in prehospital stroke assessment to improve public recognition of the disease. Besides,constructing the green channel of central retinal artery occlusion,speeding up the pre-diagnosis triage,diagnosis and treatment,shortening the time from door to needle were recommended to provide reference for improving the medical delay of patients with central retinal artery occlusion in the future.
