    A study of teaching reform in the Advanced Health Assessment Course for master of nursing specialist

    Objective To innovate the teaching design of the Advanced Health Assessment course,and to promote the cultivation of clinical practice ability and civic and political literacy of students of master of nursing specialist. Methods This study was conducted for two consecutive sessions of an advanced health assessment course,in which 91 nursing master’s degree students of a university in the grades of 2021 and 2022 were taught in an "independent-cooperative-inquiry-reflective" innovative teaching style. Data from students’ submitted reflection reports were analyzed to evaluate the effectiveness of their teaching. Results A total of 364 reflection reports were submitted by 91 students,and three themes were condensed:consolidation of professional knowledge and skills;enhancement of comprehensive competence(improvement of independent learning ability,enhancement of clinical inquiry and critical thinking ability,strengthening of teamwork awareness,and change of thinking style);and enhancement of literacy(feeling of responsibility,promotion of professional identity,enhancement of scientific research interest,enhancement of ethical literacy,and enhancement of humanistic literacy). In addition,some students also indicated that there were problems such as insufficient communication skills,clinical adaptability to be improved,and health education skills to be strengthened. Conclusion The innovative teaching of Advanced Health Assessment course cultivates the comprehensive clinical practice ability of nursing master’s degree students through autonomous,cooperative and inquiry teaching design,and further guides students to conduct in-depth thinking and rumination through reflection teaching design to sublimate the students’ civic and political literacy.

    Objective To construct a clinical teaching base extra-Beijing for special training in nursing management based on the structure-process-outcome quality evaluation model,in order to provide reference for the construction and development of nursing inservice training bases. Methods The structure-process-outcome three-dimensional quality evaluation model was used as the theoretical framework. Then the base was designed from three aspects:construction and preparation of nursing management base,training model innovation and teaching process supervision,and multidimensional assessment and evaluation. Results A total of 9 nurses have been trained,the average satisfaction of them was above 99%. The evaluation satisfaction score of the Chinese Nursing Association trainees was above 90%. Conclusion The construction of clinical teaching base extra-Beijing for special training in nursing management is scientific and reasonable,and it can provide reference for high-quality nursing management talents.

    Study on the status quo and influencing factors of reflective ability of wound ostomy and continence specialty nurses

    Objective To understand the current situation of reflection ability of wound ostomy and continence specialty nurses,and to explore its influencing factors,so as to provide reference for improving their reflection ability. Methods During April-May 2023,193 wound ostomy and continence specialty nurses from Henan Province were selected as the research objects by convenience sampling method,and the general information questionnaire,clinical nurse reflection ability scale and nurse professional identity rating scale were used for investigation. Results The score of reflective ability of specialty nurses was(89.93±12.34),and the total score of professional identity was(111.60±16.68). There was a moderately positive correlation between them(r=0.528,P<0.001). Multiple linear regression analysis showed that the working years of specialist nursing,whether they loved to reflect on wound ostomy specialty nursing work,and professional identity were the main influencing factors of the reflective ability,which could explain the 61.2% variability of the reflective ability of wound ostomy and continence specialty nurses. Conclusion The reflection ability of wound ostomy and continence specialty nurses needs to be improved. We suggest that targeted reflective ability improvement strategies should be formulated according to the characteristics of their professional working years,and their professional identity should be enhanced,so as to improve their reflective ability.

    Objective To investigate the working and career development needs of diabetes specialty nurses in Beijing, so as to provide reference for clarifying the standardized management program for diabetes specialty nurses and promoting the sustainable development of the profession. Methods A self-made questionnaire was used to investigate 536 diabetes specialty nurses(general information,working status and career development needs) in October 2022. Results A total of 536 nurses were recruited. Among them,(424,79.10%) have engaged in clinical nursing work,diabetes education(231,43.10%),nursing teaching(231,43.10%),nursing researches(89,16.60%),nursing consultations(178,33.21%),nursing clinic(138,25.75%). Regarding the professional development needs of diabetes specialty nurses,62.69%(336) believed that the establishment of full-time posts was the most urgent need for professional development at present,followed by the hope of getting support from hospital or department leaders (345,64.37%),and the hope of providing opportunities to participate in continuing education and training(336,62.69%). Conclusion It is necessary to improve the standardized management system for diabetes specialty nurses,so as to provide all aspects of supports.

    Meta-synthesis of qualitative research on professional nurse training experience in China

    Objective To systematically evaluate the real experience of our country’s specialty nurses when they receive the training of specialty nurses. Methods Qualitative studies on the training experience of specialty nurses were searched from PubMed,Web of Science,Cochrane Library,CNKI,Wanfang Database,VIP and China Biomedical Literature Database from the establishment of the database to December 2022. After literature screening and data extraction by two researchers independently,the quality of literature was evaluated using the Quality Evaluation Criteria of Evidence-Based Health Care Center of Joanna Briggs Institute,Australia(2016),and the results were integrated by Meta-synthesis method. Results Twelve literatures were included,31 research results were extracted and classified into 8 categories. Three results were obtained:1) specialized training benefited specialty nurses a lot;2) the feelings of disparity caused by the defects of the training program and its implementation coexist with feelings of self-efficacy due to the support received from various sources;3) changes in the experience of the training process-from the initial period of cherishing and full of expectations,to the progress of the emergence of a variety of negative psychological experiences,and then to the later stage of a good state of mind and the coexistence of a sense of confusion. Conclusion The nursing staffs benefit a lot from the training of specialty nurses,but the training system still has defects and needs to be further improved. At the same time,we should pay attention to the pressure faced by specialty trainees during the training,strengthen the social support system,so as to mitigate the adverse effects of negative experiences.

    Objective To explore a curriculum reform of Nursing Comprehensive Training based on the concept of "full integration of soft and hard skills",drawing on the project of the National Vocational College Nursing Skills Competition. Methods Convenient sampling was employed with students in three classes of grade 2020 as the experimental group(n=136) and students in another three classes as the control group(n=138). The teaching content of the experimental group was reformed based on the national vocational college nursing skills competition project,and online and offline mixed teaching based on teamwork were adopted. In teaching,while students’ hard skills were valued,soft skills such as communication,teamwork,problem-solving,and clinical thinking were also focused and trained. Meanwhile,traditional teaching methods were employed in the control group. The scores of OSCE(objective structured clinical examination) and the evaluations of the course were compared between the two groups. And the reflection diaries submitted by students in experimental group were analyzed to understand their learning experience. Results The OSCE scores of the experimental group[91.50(90.50,93.00)] were significantly better than those in the control group[82.25(80.00,84.50)](P<0.001). The students in the experimental group were significantly rated higher than those in the control group in the recognition of improving their practice confidence and nine types of soft and hard skills in the course(P<0.05). Besides,the analysis of the reflective reports showed that students believed that this course had strong practicality and comprehensiveness with some difficulties and challenges,stimulated their learning initiative,and enhanced their comprehensive abilities and professional confidence. Conclusion The Nursing Comprehensive Training course based on Nursing Skills Competition effectively improves students’ hard and soft skills,which is conducive to the transition training from school learning to clinical practice.

    Curriculum design and practice of geriatric care practice in higher vocational colleges based on second classroom activities

    Objective To design the second classroom of geriatric nursing in higher vocational colleges and explore the implementation effect. Methods The literatures analysis and Delphi method was used to determine the content of the second classroom of geriatric nursing in higher vocational colleges,which took the traditional solar term and disease day as the time node,and carried out science popularization of disease,health assessment,skill demonstration and recreational activities for the elderly in communities and pension institutions. A total of 76 students who selected the course were taught. Before and after the teaching,the Kogan’s Attitudes towards Old People Scale,the Geriatric Nursing Career Motivation Questionnaire,the students’ comprehensive scores and course satisfaction were used to compare. Through the text analysis of the students’ reflection report on the course activities,the teaching effectiveness was clarified. Results The course score of 76 students was (90.12±1.67) points. At the end of the course,the total score of the students’ attitude towards the elderly was (143.38±15.74) points. The value and expectation dimensions of the motivation of choosing a job in geriatric nursing were(59.30 ± 3.01) and(25.84±2.85) points,respectively. The scores were significantly higher than those before teaching(129.49±15.91,51.19±4.43,19.92±3.24)(P<0.05). The analysis of students’ reflection report classified five themes namely,consolidated the knowledge and skills learned in the first classroom,enhanced the interest in learning,enhanced the psychological quality and interpersonal communication ability,changed the stereotype effect on the elderly,and deepened the post cognition and professional identity. Conclusion The construction of the second classroom of geriatric nursing in higher vocational colleges is scientific and reasonable,which consolidates the learning of the first classroom,and students are highly satisfied with the course. It is helpful in improving students’ attitude towards the elderly and enhanced students’ willingness to engage in geriatric nursing.

    Objective To explore the application effect of escape room teaching in intensive training of medical nursing unit in higher vocational colleges. Methods Convenient sampling was used and students in two classes of nursing specialty of grade 2021 were recruited as the experimental group and students in another two classes of nursing specialty of grade 2020 were recruited as the control group. While the control group adopted the traditional unit intensive training mode,the experimental group implemented the escape room teaching on this basis. At the end of the semester,the learning engagement,final examination results and level of critical thinking ability of the two groups of students were compared. Results The scores and total scores of learning engagement,critical thinking ability,theoretical knowledge and practical skills of nursing students in the experimental group were significantly higher than those in the control group(P<0.05). Conclusion The escape room teaching can be applied in the intensive training of medical nursing unit in higher vocational colleges,which is proved to be effect in stimulating the learning input of nursing students,improving the level of knowledge and skills,and cultivating the critical thinking ability of nursing students.

    Study on the application effect of intelligent teaching platform combined with video feedback teaching method in community health service course of connecting nursing specialty in secondary and higher vocational schools

    Objective To explore the application effect of the intelligent teaching platform combined with video feedback teaching method in the community health service course for the transition from secondary vocational education to higher vocational nursing profession. Methods In the second semester of the 2020—2021 academic year,two parallel classes of 2020 secondary and higher vocational nursing students were selected by random sampling. The two classes were randomly divided into the experimental group(n=61) and the control group(n=64) by flipping a coin. Students in the experimental group received the intelligent teaching platform combined with video feedback teaching method in the community health service course,while students in the control group received the traditional teaching method. After the completion of the teaching,a comparison was made between the two groups in terms of their self-learning ability,theoretical examination scores,practical operation assessment scores,and evaluation of the effectiveness of the teaching method. Results After the teaching,the experimental group showed significantly higher scores in self-learning ability score (123.95±4.25) compared to the control group (97.11±5.64)(t=29.952,P<0.001). Students in the experimental group achieved better theoretical examination scores,assessment scores in five practical operation items,and evaluation results on the effectiveness of teaching methods compared to the control group(all P<0.05). Conclusion The intelligent teaching platform combined with video feedback teaching method has a good application effect in the teaching of community health service for secondary and higher vocational nursing students.

    Objective To explore the teaching practice and its effect of ideological and political education in the Fundamentals of Nursing. Methods The following measures had been taken to combine ideological and political elements with professional knowledge:clarifying curriculum ideological and political objectives,developing teaching design,constructing ideological and political teaching resource base,strengthening the construction of ideological and political teachers. From September 2021 to July 2022,a total of 118 second year nursing undergraduates were recruited as participants. After implementation,a course satisfaction questionnaire was used to evaluate students’ satisfaction with ideological and political education,impact on their willingness to engage in nursing work,and understand their learning experience. Results The overall satisfaction of students about ideological and political education in the Fundamentals of Nursing was 92%(109/118),with satisfaction rate of the permeation of various ideological and political elements between 75%-85%. Sixty-five students(55%) expressed stronger will to engage in nursing work. 93%(110/118) of the students reported that they gained something from the course’s ideological and political teaching. The course has been recognized as a top-notch provincial online and offline blended course. Conclusion The ideological and political education in the Fundamentals of Nursing has been affirmed by students,which is helpful to enhance students’ professional identity and ideological and political literacy,improve teachers’ ability,and promote to achieve the goal of "trinity" of value guidance,ability training and knowledge imparts.

    Development and evaluation of an OSCE case base for surgical nursing practice of interns

    Objective Based on the construction an OSCE case base for the teaching of surgical nursing practice based on the Association for Medical Education in Europe Guide No. 81(AMEE Guide No. 81),this study aims to explore its usage for standardizing the construction of an OSCE case base for teaching surgical practice in the future. Methods Based on the AMEE Guide No. 81 guidance,the case library construction was carried out from December 2021 to January 2023,including the formation of an expert review panel,the drawing of an authoring framework,the identification of authoring topics,the selection of an authoring team,and the development of a case library template. Then,we invited 19 experts for two rounds of correspondence using the Delphi method to revise the OSCE case base based on the experts’ opinions. Finally,we analyzed the results of the case quality scores from the 2nd round of consultation. Results The study has developed a surgical practice teaching OSCE case base consisting of 16 categories of surgical nursing practice teaching OSCE specialty cases. Each category of case was based on a developed authoring template that included six specific sections:information about the examination station,candidate guidelines,examiner guidelines,standardized patient instructions,assessment supplies and equipment,and other information about the case. Quality scores for each case were ranged from 87.63 to 91.47. Conclusion Based on AMEE Guide No. 81,a high quality OSCE case bank for teaching surgical nursing practice is suitable for the teaching in surgical nursing clinical practice.

    The mediating effects of psychological capital and organizational commitment on the relationships between calling and work engagement among clinical nurses

    Objective To explore the mediating role of psychological capital and organizational commitment between calling and work engagement of clinical nurses. Methods The convenience sampling method was used. From June 2022 to July 2022,633 clinical nurses from three tertiary hospitals in Guangzhou were selected as the research objects. General information questionnaire,Calling Scale,Psychological Capital Scale,Organizational Commitment Scale,Work Engagement Scale were used to investigate. Results The calling of clinical nurses was scored as (3.69±0.73),psychological capital was scored as (4.03±0.62),organizational commitment was scored as (2.70±0.54),work engagement was scored as(3.93±1.22). There were positive correlations among the four variables(P<0.001). The mediation model revealed that the direct effect of calling on work engagement was significant(β=0.269,P<0.001). Besides,calling could also indirectly affect work engagement through three pathways:the independent mediating effect of psychological capital(β=0.112,P<0.001),the independent mediating effect of organizational commitment(β=0.122,P<0.001),and the chain mediating effect of psychological capital and organizational commitment(β=0.038,P<0.001),with the mediating effects accounting for 50.28% of the total effects. Conclusion Psychological capital and organizational commitment play a mediating role between clinical nurses’ calling and work engagement,and their work engagement can be increased by improving clinical nurses’ psychological capital and strengthening organizational commitment.

    Translation and psychometric properties evaluation of the Chinese version of Standard Precautions Questionnaire

    Objective To translate the English version of the Standard Precautions Questionnaire(SPQ) into Chinese and analyze its reliability and validity. Methods After obtaining authorization to utilize the SPQ,the English version of the SPQ was translated and adapted culturally,and the Chinese version of the SPQ was formed after validity testing of the questionnaire content by experts,pretesting,and applicability evaluation. The Chinese version of the SPQ reliability and validity test was conducted from December 2021 to March 2022 using convenience sampling among a total of 609 clinical nursing staffs in three tertiary level A general hospitals in Sichuan Province. Results The Chinese version of the SPQ content validity index was 0.750-1.000 for each item level and 0.916 for the questionnaire. The exploratory factor analysis extracted a total of 7 factors,which were consistent with the original scale,and the cumulative variance contribution to the scale was 80.784%. The load value of each item on the common factor was 0.770-0.904. The Confirmatory factor analysis χ2/df=1.470,RMSEA=0.039,GFI=0.963,AGFI=0.940,CFI=0.985,IFI=0.985,TLI=0.980,NFI=0.956 with good model fit indicators. The Cronbach’s α of the total questionnaire was 0.894,and the Cronbach’s α of each dimension was distributed between 0.836-0.950. The test-retest reliablity coefficient of the questionnaire was 0.989. The intra-group correlation coefficient of the retest reliability of each dimension was well distributed between 0.900-0.989(P<0.05). Conclusion The Chinese version of SPQ has good reliability and validity. It can be used as an assessment instrument for nurses’ compliance with standard precautions and impact factors.

    Validation of Chinese version of Patient Participation Culture Tool for Healthcare Workers and its application in clinical nurses

    Objective The purpose of this study was to translate and revise Patient Participation Culture Tool for Healthcare Workers(PaCT-HCW) in Chinese background,so as to provide a reliable tool for evaluating the level of patient engagement. Methods The translation and back-translation of PaCT-HCW were followed by the Brislin translation model. The Chinese version was culturally adjusted and refined through expert consultations and pre-investigation. Content validity of the Chinese version was evaluated by seven experts. A questionnaire survey was conducted among 812 nurses. Fifty clinical nurses were selected to investigate again after two weeks. Results The Chinese version of PaCT-HCW consisted of 44 items into six dimensions:competence,support,perceived lack of time,information sharing and dialogue,response to questions,and acceptance of a new role. Exploratory factor analysis revealed six common factors,accounting for a cumulative variance contribution rate of 69.829%. The item content validity index(I-CVI) of the scale rangedfrom 0.710 to 1.000,and average content validity(S-CVI/Ave) reached 0.970. The Cronbach’s α for the whole scale was 0.962,while varied from 0.748 to 0.956 for dimensions. The test-retest reliability of the scale was 0.980,andranged from 0.690 to 0.977 for dimensions. Conclusion The Chinese version of PaCT-HCW has good reliability and validity,and can be used as a valid tool in evaluating the level of patients’ participation cultural for nurses.

    Best evidence summary of exercise intervention in patients with liver cirrhosis and sarcopenia

    Objective To search,evaluate and summarize the best evidence for exercise intervention in patients with liver cirrhosis and sarcopenia,and to provide reference for health care personnel. Methods To retrieve the literature on exercise intervention in patients with cirrhosis and sarcopenia,a computer-based search was undertaken in databases including UpToDate,BMJ,JBI,Cochrane Library,Web of Science,PubMed,Embase,CNKI,Wanfang Data,VIP,CBM and other databases. The search time limit was set from the establishment of the databases to May 2,2023. Results A total of 10 articles were included,including two guidelines,two expert consensus,and six systematic reviews. Finally,21 best evidences in six aspects including pre-exercise evaluation,exercise mode,exercise intensity,exercise frequency and time,exercise monitoring and quality control were summarized for patients with cirrhosis and sarcopenia. Conclusion This study summarized the best evidence for exercise intervention in patients with liver cirrhosis and sarcopenia. It is recommended that health care personnel should evaluate patients before exercise,select individualized exercise methods,intensity,frequency and time,and dynamically monitor the patients’ disease progression,muscle strength,quality,body function and other related indicators to ensure the safety and effectiveness of exercise intervention and improve the patients’ activity endurance as well as quality of life.

    Objective To summarize the best evidence of exercise programs for children with juvenile idiopathic arthritis and to provide basis for clinical exercise plans. Methods According to the evidence pyramid model,relevant papers from the national and international databases and the relevant websites were searched from databases’ establishment to October 2022. The quality of the literature was evaluated and the extracted evidence was graded through the literature evaluation criteria and evidence classification system of JBI Evidence-based Health Care Center in Australia. Results A total of ten publications including two guidelines,two expert reports and six systematic reviews were included. A total of 28 pieces of evidence were identified and categorized into eight aspects:exercise principle,exercise assessment,exercise type,exercise intensity,exercise frequency and time,exercise safty,exercise managements and education. Conclusion This study summarized the best evidence on exercise programs for children with juvenile idiopathic arthritis. Health professionals can refer to the best evidence and combine it with the child’s own condition and the wishes of the child or parents in order to develop an individualized exercise programs to improve the child’s exercise capacity and quality of life.

    Objective To investigate the status of disability acceptance in patients with spinal cord injury and analyze its influencing factors. Methods A total of 161 patients with spinal cord injury admitted to a Grade-A hospital in Beijing from January 2019 to December 2022 were selected by convenience sampling method. General data questionnaire,Revised Acceptance Disability Scale and Chinese version of Herth Hope Index were used to investigate the patients. Results The total score of disability acceptance in patients with spinal cord injury was(70.48±14.76). Injury lesion,injury duration and hope level could explain 39.4% variation in disability acceptance. Conclusion The disability acceptance of patients with spinal cord injury is at a medium low level of acceptance. Injury lesion,injury duration and hope level are the influencing factors of disability acceptance. Medical staffs can develop intervention programs to improve disability acceptance of patients with spinal cord injury according to the characteristics of different injury lesion,injury duration and hope level,eventually improving the psychological condition,and rehabilitation adherence and efficacy of patients.

    Objective To systematically review the fall prediction and monitor value of machine learning (ML) for older adults. Methods Databases including CNKI,SinoMed,VIP,Wanfang Data,Web of Science,PubMed,Embase and CINAHL were searched to retrieve all studies that focused on ML in predicting fall of older adults. The searching time was set from the date of database’s establishment to July 2022. After that,two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies by the standard of QUADAS-2. Results A total of 15 studies were included. In most studies,neural network and random forest models were mainly being used. In addition,though a few studies focused on specific populations,such as older women with osteoporosis and older patients in acute care,the majority of studies included older adults. Based on the quality evaluation,the included studies were highly applicable and had a low risk of bias. Conclusion ML has high predictive value and application value in fall identification and fall risk prediction for older adults. Meanwhile,there are still some problems. In the future,big data analysis should still base on clinical experience,as well as pay attention to specific populations,in order to develop a deeper combination and development of artificial intelligence and nursing.

    ChatGPT (Chat Generative Pre-trained Transformer),as a large language model of artificial intelligence (AI) based on dialogue,has gained strong momentum in its development. It is likely to offer conveniences to nursing field,but with it comes questions about ethics,prejudice,privacy and other aspects. This paper aims to collect the related articles in the research field of ChatGPT as well as their application data in the nursing field,and summarize the condition of applied research and potential risks of ChatGPT in combination with nursing related scenes,so as to provide reference for the application of AI products in nursing education,nursing research and clinical nursing in China.

    Cardiovascular disease is the leading cause of death in China,and interventional therapy,as an important treatment,is widely used. Patients after cardiac interventional therapy should start exercise rehabilitation programme as soon as possible to improve prognosis. Kinesiophobia after cardiac intervention is common and has become an key hindrance for patients to participate in cardiac rehabilitation. In addition,kinesiophobia can cause anxiety,depression and other negative emotions,leading to a decline in the quality of patients’ life. This paper reviewed the prevalence,assessment tools,influencing factors and intervention measures of kinesiophobia,so as to provide science foundation for reducing patients’ kinesiophobia after cardiac intervention.
