摘要:

目的 探究本科护理实习生转型冲击的变化轨迹及临床实践自我调节学习能力对其的影响。 方法 于2024年8月—2025年3月选取正在进行首次临床毕业实习的本科护理实习生为研究对象。使用一般资料调查表、中文版护理本科实习生转型冲击评价量表、中文版护生临床护理实践自我调节学习量表、主动性人格量表在实习前、中、后期进行3次追踪测试,调查时间间隔3个月。采用潜变量增长模型进行数据分析。 结果 本科护理实习生转型冲击(截距=44.726,P<0.001)在3次测量下随时间推移呈线性上升趋势(斜率=0.762,P<0.001)。在控制主动性人格下,纳入临床护理实践自我调节学习能力为随时间变化协变量,发现3个实习阶段临床护理实践自我调节学习能力均负向预测转型冲击(P<0.001)。 结论 本科护理实习生转型冲击呈逐渐上升趋势,临床护理实践自我调节学习能力是转型冲击发展过程中的抑制因素。建议护理教育者通过多元途径促进本科护理实习生临床护理实践自我调节学习能力的提高以应对转型冲击,增强学生临床实践效果。

Abstract:

Objective To explore the change trajectory of transition shock among undergraduate nursing interns and the influence of self-regulated learning ability in clinical practice. Methods Undergraduate nursing students undergoing their first clinical internship were recruited from August 2024 to March 2025. Data were collected at three time points(early stage,mid stage,and late stage of internship) with three-month intervals using the General Information Questionnaire,Chinese version of the Transition Shock Evaluation Scale for Nursing Undergraduate Interns,Chinese version of the Self-Regulated Learning Scale in Clinical Nursing Practice,and Proactive Personality Scale. Latent growth model was employed for data analysis. Results Transition shock of undergraduate nursing interns demonstrated a significant linear upward trajectory across the three measurements time points(intercept=44.726,P<0.001;slope=0.762,P<0.001). After controlling for proactive personality,the inclusion of self-regulated learning ability in clinical nursing practice as a time-varying covariate revealed its consistent negative predictive effects on transition shock across all three internship phases(P<0.001). Conclusion Transition shock progressively intensifies during clinical internships,while self-regulated learning ability in clinical practice serves as an inhibiting factor in this developmental process. Nursing educators should implement multifaceted strategies to enhance undergraduates’ self-regulated learning ability in clinical settings,thereby mitigating transition shock and optimizing clinical performance.
