缺血性卒中患者静脉血栓预防及管理的证据总结

摘要：

目的总结、评价国内外关于缺血性卒中患者静脉血栓预防及管理的证据,为临床护理人员提供循证依据。方法检索UpToDate、BMJ best practice、Cochrane、苏格兰学院间指南网、中国生物医学文献数据库、万方数据库等国内外数据库,检索时间限制为建库至2019年12月5日,由2名研究者使用AGREE Ⅱ评价指南质量,使用JBI循证卫生保健中心对应的评价标准（2016）对系统评价进行质量评价,并进行证据提取。结果本研究共纳入了18篇文献,包括15篇指南,2篇系统评价和1篇证据综合,共提取证据30条,主要内容包括缺血性卒中患者静脉血栓发生的风险评估、间歇压力充气泵预防、抗凝药预防及其他预防措施的实施方法和及治疗方法。结论本研究总结了缺血性卒中患者静脉血栓预防及管理的证据,能够为临床护理工作人员对缺血性卒中患者静脉血栓的预防及管理提供科学的依据,由于本研究总结的证据来自多个国家,在应用前应进行充分的临床环境及其他相关因素的评估,以促进护理质量的提升。

Abstract:

Objective To summarize and evaluate the evidence for the prevention and management of venous thromboembolism in patients with ischemic stroke. Methods We searched evidence in UpToDate,BMJ best practice,the Cochrane,Scottish Intercollegiate Guidelines Network,China Biology Medicine disc,and Wanfang database from inception to December 5,2019. The quality assessment was conducted by two researchers using AGREEⅡ and JBI evidence-based health care center（2016） evaluate standards for systematic review. Results A total of 18 articles were included in this study,including 15 guidelines,2 systematic reviews and 1 evidence summary. Thirty pieces of the best evidence were extracted,including the risk assessment of venous thrombosis in patients with ischemic stroke,intermittent pneumatic compression prevention,anticoagulant prevention and other preventive and treatment measures. Conclusion This study summarized the evidence of the vein thrombosis prevention and management of patients with ischemic stroke and it can provide reference for clinical practice. In order to promote nursing quality,clinical environment and other related factors should be assessed before application due to the evidence from various countries.
