Abstract:

Objective To understand the current status and influencing factors of knowledge, attitude and practice (KAP) of transitional care of nurses working in cardiovascular nursing field in Fujian Province. Methods A convenience sampling was used to recruit 447 nurses working in cardiovascular nursing field in 60 hospitals of Fujian Province. The nurses were investigated by the General Information Questionnaire, the Transitional Care Knowledge,Attitude and Practice Scale and the Influencing Factors of Transitional Care Practice. Results The total score of KAP of the 447 cardiovascular nurses was (146.96±22.13),with a mean item score of (3.95±0.80). Subscale scores included knowledge(3.72±0.88),attitudes(4.08±0.72),and practices(3.77±0.99). Significant differences regarding the total KAP scores were observed across age, professional title, position, hospital level, and whether the nurse held a specialized certification(all P<0.05). The main factors that affected the practice of transitional care included the presence of transitional care system and procedures, clearly defined roles and responsibilities, and information sharing and collaboration between the hospital and other referral agencies. Conclusion The level of KAP of transitional care among nurses working in cardiovascular nursing field in Fujian Province requires further improvement. Managers ought to further improve the appropriate systems and processes for transitional care, and strengthen cooperation between disciplines and between healthcare organizations, and promote the high quality transitional care services.
