Abstract:

Objective To explore the effects of the follow-up process based on a case management model in cerebrovascular patients with stents. Methods Patients who were admitted in the neurology department of a hospital for stents treatment from June to December 2020 were recruited in the study. According to the location of the department(Neurology Area A and Area B),patients were assigned to the interventional group(120 cases) and the control group(121 cases). Patients in the interventional group received follow-up based on a case management model while patients in the control group received routine follow-up. The outpatient clinic revisit rate, waiting time in outpatient clinic, and patients' satisfaction were compared between the two groups in 6 months after discharge. Results The outpatient clinic revisit rate of patients in the interventional group was significantly higher than that of patients in the control group(94.58% vs 72.64%, χ2=64.941,P<0.001). The medica-tion adherence of patients in the interventional group[7.25(7.00,7.86)] was higher than that in the control group[7.25(5.25,7.56)](U=5 468,P<0.001). The waiting time in the outpatient clinic in the trial group was significantly lower than that in the control group[115(105.25,123.00) min vs 166(154.5,178.00)min](U=14 510,P<0.001). Patients' satisfaction level in the interventional group was higher than that in the control group[9.25(9.00,10.00) vs [9.00(8.75,9.25)](U=4402,P<0.001). Conclusion The follow-up process based on a case mana-gement model can improve the revisit rate in the outpatient clinic, medication adherence, reduced outpatient waiting time, and satisfaction level of patients.
