咸宁市医养结合机构入住老年人对服务推荐度的调查研究

摘要：

目的了解咸宁市入住医养结合机构老年人对服务推荐度现状，为提升医养结合服务质量提供依据。方法采取方便抽样法，选取咸宁市11所市级医养结合示范机构的216名入住老年人作为研究对象，采用自设问卷对入住老年人的一般资料、医养结合服务了解度、满意度、推荐度状况进行调查。结果入住老年人对医养结合服务推荐的有163名（75.5%），一般推荐的有42名（19.4%），不推荐的有11名（5.1%）。单因素分析结果显示，不同配偶情况的老年人对医养结合服务推荐度的差异具有统计学意义（Z=2.379，P=0.017）；入住老年人医养结合服务了解度（r=0.242，P<0.001）、机构服务整体满意度（r=0.169，P=0.013）、医疗服务满意度（r=0.309，P<0.001）、饮食情况满意度（r=0.304，P<0.001）、机构环境及硬件设施满意度（r=0.195，P=0.004）、工作人员服务水平满意度（r=0.198，P=0.003）均与医养结合服务推荐度呈正相关。结论咸宁市医养结合机构入住老年人对医养结合服务的推荐度较高。建议加大政策支持，同时加强宣传，以改善老年人传统观念。与此同时，机构需进一步提升医疗服务供给能力和老年人饮食健康管理能力，更加注重老年人心理慰藉服务，从而不断提高医养结合服务能力。

Abstract:

Objective To investigate the current situation of the recommendation degree of the combined medical and elderly care services for the older adults living in the institutions in Xianning City and in order to provide reference for improving the quality of the services. Methods A total of 216 elderly residents from 11 municipal exemplary the combined medical and elderly care institutions in Xianning City were recruited by convenience sampling. A self-designed questionnaire was used to investigate the general information，the understanding，satisfaction and recommendation degree of the elderly living in these institutions. Results Among 216 elderly residents，163（75.5%） recommended the combination of medical and elderly care services，42（19.4%） generally recommend it，and 11（5.1%） did not recommend it. Single factor analysis showed the comparison of the recommendation degree of elderly people was significant different among participants with spouse or not（Z=2.379，P=0.017）. Besides，a number of factors were positively correlated with the recommendation degree of the combined medical and elderly care services for the elderly living in the institutions including the understanding degree of the combined medical and elderly care services（r=0.242，P<0.001），the satisfaction degree of institutional overall services（r=0.169，P=0.013），the satisfaction degree of medical services（r=0.309，P<0.001），dietary status（r=0.304，P<0.001），institutional environment，hardware facilities（r=0.195，P=0.004） and staff service levels（r=0.198，P=0.003）. Conclusion The recommendation degree of the combined medical and elderly care services for elderly living in the institutions in Xianning City is relatively high. It is recommended to increase policy support，strengthen publicity to improve the traditional concepts of the elderly. At the same time，institutions need to further enhance the supply of medical services and the diet health management ability of the elderly，and pay more attention to the psychological comfort services of the elderly，so as to continuously improve the quality of the combined medical and elderly care services.
