多成分干预在提高儿童对颅脑磁共振成像检查配合度中的应用研究

Application of multi-component intervention in improving children’s compliance with cranial MRI examination

摘要:

目的 探讨多成分干预在提高3~9岁儿童对颅脑磁共振成像(Magnetic Resonance Imaging,MRI)检查配合度中的效果。方法 选取2024年3月—10月于湖南省儿童医院放射科接受首次非镇静性颅脑MRI检查的3~9岁患儿作为研究对象,根据家长对本研究的参与意愿情况,将患儿分入对照组和试验组。对照组接受常规护理,试验组根据患儿年龄(3~6岁/>6~9岁)接受差异化的多成分干预,包含环境适应训练、认知行为重塑、沉浸式游戏互动及视听分散等,比较两组的检查一次性成功率、检查时长及镇静使用率。结果 试验组与对照组均包含3~6岁、>6~9岁患儿各50例。试验组3~6岁患儿检查一次性成功率、检查时长及镇静使用率均优于对照组3~6岁患儿(均P<0.05)。试验组>6~9岁患儿检查时长短于对照组>6~9岁患儿(P<0.001),但两组>6~9岁患儿检查一次性成功率及镇静使用率比较,差异无统计学意义(P>0.05)。结论 多成分干预对于3~9岁接受首次非镇静性颅脑MRI检查的患儿可提升其对检查的配合度,并能减少3~6岁者对镇静的需求,具有一定的推广价值。

Abstract:

Objective To explore the effect of multi-component intervention in improving children’s compliance with cranial Magnetic Resonance Imaging(MRI) examination. Methods From March to October 2024,children aged 3-9 years old who underwent their first non-sedative cranial MRI examination in the Radiology Department of Hunan Children’s Hospital were selected as the participants. Children were divided into a control group and an experimental group according to their willingness. The control group received routine nursing care,while the experimental group received differentiated multi-component intervention(including environmental adaptation training,cognitive-behavioral remodeling,immersive interactive games,and audio-visual distraction) based on the children’s age(3-6 years/>6-9 years). The one-time success rate of the examination,examination duration,and sedation usage rate were compared between the two groups. Results Both the experimental group and the control group included 50 children aged 3-6 years old and 50 children aged >6-9 years old,respectively. For children aged 3-6 years old,the one-time success rate of the examination,examination duration,and sedation usage rate in the experimental group were all better than those in the control group(P<0.05). For children aged >6-9 years old,the examination duration in the experimental group was shorter than that in the control group(P<0.001). However,there was no statistically significant difference in the one-time success rate of the examination and sedation usage rate between the two groups(P>0.05). Conclusion Multi-component intervention can improve the compliance of children aged 3-9 years old undergoing their first non-sedative cranial MRI examination and reduce the need for sedation in children aged 3-6 years old,which holds certain promotion value.
