Summary of the best evidence on discharge preparation service for patients with chronic obstructive pulmonary disease

Abstract:

Objective To search,evaluate and summarize the best evidence of discharge preparation service for patients with COPD,and to provide reference for clinical nursing practice. Methods The literatures related to the discharge preparation service of COPD patients in relevant websites and domestic and foreign databases were systematically searched,including guidelines,expert consensus,evidence summaries,systematic reviews,best clinical practices,standards. Two researchers finished the quality evaluation of the relevant literatures,extracted and summarized the evidence according to the theme in combination with professional judgment. Results A total of 13 articles were included,including 6 guidelines,1 best practice,5 systematic reviews,1 evidence summaries. Thirty pieces of the best evidence on COPD patients’ discharge preparation services were formed from six aspects:screening and evaluation,planning,health guidance during hospitalization,preparation before discharge,discharge guidance,and post-discharge and evaluation follow-up. Conclusion This study summarizes the best evidence for the implementation of discharge preparation service for patients with COPD,which can provide evidence-based basis for medical staffs to formulate reasonable discharge preparation service plan,standardize clinical practice and ensure the safety of post-discharge transition.
