Objective To evaluate the effects of case management model in the health management of patients with type 2 diabetes mellitus. Methods Totally 152 diabetic patients were divided into two groups according to the wards of department of endocrinology. Case management model and routine health management model was applied in the experimental group and the control group，respectively. All patients were investigated using the self-manage-ment ability questionnaire before and after intervention. Moreover，the fasting blood glucose，2h postprandial blood glucose and glycosylated hemoglobin were compared between the two groups. Results After intervention，the factor scores of dietary control，foot care and coping with high-low blood glucose management were significantly higher in the experimental group than those of the control group（P<0.05），while no significant differences were found in the total score of self-management and factor scores of sports management and drug monitoring between the two groups （P>0.05）. After intervention，the fasting blood glucose and postprandial 2h blood glucose in the experimental group were significantly lower than those of the control group（P<0.05），while no significant difference was found in the glycosylated hemoglobin between the two groups（P>0.05）. Conclusion The application of case management model in the health management of patients with type 2 diabetes mellitus can improve the self-management ability in dietary control，foot care and coping with high-low blood glucose，and enhance the control of glucose metabolism.
