Chinese version instruments of measuring fear of falling for the elderly：a systematic review of psychometric properties

摘要

目的 对中文版老年人跌倒恐惧评估工具的测量特性及方法学质量进行评价，为医护人员选择评估工具提供依据。 方法 检索PubMed、Embase、CINAHL、Web of Science、PsycInfo、中国生物医学文献数据库、中国知网、万方和维普数据库中有关中文版老年人跌倒恐惧评估工具的研究，检索时限为建库至2024年5月15日。由2名研究者独立筛选文献和提取数据，采用基于共识的健康测量工具遴选标准（Consensus-based Standards for the Selection of Health Measurement Instruments，COSMIN），对评估工具进行评价并形成最终推荐意见。 结果 共纳入17项研究，包括13种中文版老年人跌倒恐惧测量工具，均未报告工具的跨文化效度和反应度。最终养老机构跌倒自我效能量表（Nursing Home Falls Self-Efficacy Scale，NHFSS）为A类推荐，简版特定活动平衡信心量表（Short version of the Acti-vities-specific Balance Confidence Scale，ABC-6）和老年跌倒恐惧评估表（Geriatric Fear of Falling Measure，GFFM）为C类推荐，修订版跌倒恐惧问卷（Fear of Falling Questionnaire-Revised，FFQ-R）等10个评估工具为B类推荐。结论 NHFSS内容效度较好，优先推荐应用，但仅适用于养老机构老年人。FFQ-R可综合性和多维度评价老年人的跌倒恐惧，可初步推荐使用，但其测量学属性仍有待验证。

Abstract

Objective To evaluate the quality of psychometric properties and methodological quality of the Chinese version instruments for measuring fear of falling（FOF） in the elderly，and to provide reference for healthcare professionals in selecting assessment tools. Methods Databases were searched to collect studies on the Chinese version instruments for measuring FOF in the elderly since their inception to May 15，2024，including PubMed，Embase，CINAHL，Web of Science，PsycInfo，CBM，CNKI，Wanfang Data，and VIP. After that，two researchers completed literature screening and data extraction independently. The Consensus-based Standards for the Selection of Health Measurement Instruments（COSMIN） guidelines were used to evaluate the assessment tools. Finally，recommendations were made. Results A total of 17 studies were included，involving 13 Chinese version instruments for measuring FOF in the elderly. None of them reported the cross-cultural validity and responsiveness. Through comparing，NHFSS was recommended as Category A，ABC-6 and GFFM were recommended as Category C，while the other 10 scales，including FFQ-R，were recommended as Category B. Conclusion NHFSS holds good content validity and is preferentially recommended for use，though only applicable in old adults in elderly care institutions. FFQ-R，which comprehensively and multidimensionally evaluates fear of falling in the elderly，can be initially recommended，but its psychometric properties need further validation.
