Influencing factors of pulmonary rehabilitation benefit in older patients with stable chronic obstructive pulmonary disease
目的 了解老年COPD稳定期患者肺康复获益现状,并分析其影响因素,为临床促进患者肺康复获益提供参考。方法 回顾性收集2020年8月—2023年4月南通市某三级医院完成肺康复课程的173例COPD稳定期患者的临床资料,根据患者参加肺康复课程前后6分钟步行测试和圣乔治呼吸问卷得分变化情况将其分为获益组和未获益组,采用Logistic回归分析患者肺康复获益的影响因素。结果 173例完成肺康复课程的患者中,93例(53.76%)患者达到了获益。Logistic回归分析显示,年龄(OR=0.921)、肺康复前6分钟步行测试(OR=0.976)、肺康复前圣乔治呼吸问卷得分(OR=1.221)、自我效能(OR=1.381)、合并骨质疏松症(OR=0.406)是老年COPD稳定期患者肺康复获益的影响因素(P<0.05)。结论 COPD稳定期患者肺康复获益率较低,肺康复前6分钟步行测试结果越差、肺康复前圣乔治呼吸问卷得分越高、自我效能良好的患者产生肺康复获益的概率越高;年龄越高、合并骨质疏松症的患者肺康复未获益的风险较高,临床医护人员对患者进行肺康复干预时需对其获益影响因素加以考虑,完善干预方案,从而促进患者康复。

Abstract To understand the current situation of pulmonary rehabilitation benefits in stable older COPD patients and analyze its influencing factors so as to provide reference for clinical promotion of pulmonary rehabilitation benefits in patients. Methods Retrospective collection of clinical data was carried out in 173 stable COPD patients who completed lung rehabilitation courses in a tertiary hospital of Nantong City from August 2020 to April 2023. Based on the changes in 6-minute walk test and St. George’s Respiratory Questionnaire(SGRQ) scores before and after participating in lung rehabilitation courses,patients were divided into benefit group and non-benefit group. Logistic regression analysis was used to analyze the influencing factors of lung rehabilitation benefits for patients. Results Among 173 patients who completed lung rehabilitation courses,93(53.76%) achieved benefits. Logistic regression analysis showed that age(OR=0.921),baseline 6-minute walk test(OR=0.976),baseline SGRQ score (OR=1.221),self-efficacy(OR=1.381),and comorbid osteoporosis(OR=0.406) were the influencing factors for pulmonary rehabilitation benefits in stable older COPD patients(P<0.05). Conclusion Stable COPD patients have a lower rate of pulmonary rehabilitation benefits,and patients with poorer the baseline 6-minute walk test results, higher baseline SGRQ score and good self-efficacy are more likely to experience pulmonary rehabilitation benefits. The older in age and combined with osteoporosis hold higher the risk of not benefiting from pulmonary rehabilitation. Clinical medical staff need to consider the factors that affect the benefits of pulmonary rehabilitation interventions for patients,improve the intervention plan to promote the recovery of the patients.
