Development of a nursing training program for patients with post-stroke cognitive impairment

目的 构建卒中后认知障碍的护理培训方案,为护理人员开展继续教育提供参考。 方法 在文献分析基础上对临床医生、护士、康复治疗师和护理教育者进行半结构式访谈,设计卒中后认知障碍护理培训方案初稿,采用Delphi法对25名专家进行2轮专家函询后形成了卒中后认知障碍护理培训方案。 结果 2轮专家函询的积极系数分别为84.0%(21/25)、95.2%(20/21),专家权威系数分别为0.85、0.86;项目重要性肯德尔和谐系数分别为0.214、0.316(均P<0.001)。最终形成的卒中后认知障碍护理培训方案包括培训目标(4个一级项目,11个二级项目)、培训内容(10个一级项目,48个二级项目)、培训形式(7项)、培训时间(46学时)、培训师资(3项)及培训评价(4项)共6个部分。 结论 本研究构建的卒中后认知障碍护理培训方案内容充实,具有较高的可靠性和专家认可度,适用于护士培训;仍有待通过实践进行完善并评价其培训效果。

Objective To develop a nursing training program for patients with post-stroke cognitive impairment. Methods Semi-structured interviews with clinicians,nurses,rehabilitation therapists,and nursing educators were conducted on the basis of literature analysis to design a draft of the training program. The Delphi technique was used to conduct 2 rounds of expert consultations with 25 experts. Results The positive coefficients of the two rounds of expert letter inquiries were 84.0%(21/25) and 95.2%(20/21),and the expert authority coefficients were 0.85 and 0.86 respectively. The Kendall coefficients of item importance were 0.214 and 0.316 respectively(P<0.001). The final training program consisted of 6 parts including training objectives(4 first-level items,11 second-level items),training contents(10 first-level items,48 second-level items),and training form(7 items),training time(46 class hours),training teachers(3 items) and training evaluation(4 items). Conclusion The post-stroke cognitive impairment nursing training program has plenty of contents,high reliability and expert recognition,and is suitable for nurse training. The program still needs to be improved through practice and its training effects are needed to be examined.
