摘要

目的 探讨应对方式在中青年冠心病患者知觉压力与睡眠质量间的中介效应。方法 于2019年8月—2020年7月采用一般资料问卷、医学应对方式问卷、知觉压力量表及匹兹堡睡眠质量指数量表,对新乡医学院5所附属医院的242例中青年冠心病患者进行调查。结果 中青年冠心病患者知觉压力得分为（30.71±7.39）分;应对方式中,面对维度得分为（18.11±3.93）分,回避维度得分为（16.36±2.76）分,屈服维度得分为（9.86±3.10）分;睡眠质量得分为（7.44±3.60）分。知觉压力与应对方式共解释睡眠质量总变异的46.9%,应对方式在知觉压力与睡眠质量间存在中介效应（面对:0.155;回避:0.079;屈服:0.087）,P<0.05。结论 知觉压力、应对方式对中青年冠心病患者的睡眠质量有一定的预测作用,应对方式在知觉压力与睡眠质量之间起中介作用。医护人员应关注中青年冠心病患者的应对方式,采取相应的干预措施,减少其压力,提高其睡眠质量。

Abstract

Objective To investigate the mediating effect of coping style on perceived stress and sleep quality in young and middle-aged patients with coronary heart disease. Methods From August 2019 to July 2020,general information questionnaire,Medical Coping Modes Questionnaire,Perceived Stress Scale and Pittsburgh Sleep Quality Index were used to investigate 242 young and middle-aged patients with coronary heart disease in 5 affiliated hospitals of Xinxiang Medical University. Results The total score of perceived stress in young and middle-aged patients with coronary heart disease was（30.71±7.39）. The score of confrontation dimension of coping mode was（18.11±3.93）,avoidance dimension was（16.36±2.76）,and resignation dimension was（9.86±3.10）. The sleep quality score was（7.44±3.60）. Perceived stress and coping mode explained 46.9% of the total variation of sleep quality,and coping mode mediated the relationship between perceived stress and sleep quality（confrontation:0.155;avoidance:0.079;resignation:0.087）,P<0.05. Conclusion Perceived stress and coping mode can predict sleep quality in young and middle-aged patients with coronary heart disease,and coping mode plays a mediating role between perceived stress and sleep quality. Medical staff should pay attention to the coping mode of young and middle-aged patients with coronary heart disease and evaluate the coping mode of hospitalized patients,in order to take targeted measures to reduce their stress and improve their sleep quality.
