2型糖尿病患者个案管理的实践研究

摘要: 目的 探讨个案管理模式在2型糖尿病患者健康管理中的效果。 方法 按照病区将内分泌病房患者分为2组,每组76例,1个病区为试验组,对其进行个案管理;1个病区为对照组,采用常规健康管理。 在干预前后用自我管理能力调查问卷对患者进行调查,并检测空腹血糖、餐后2h血糖及糖化血红蛋白。 结果 干预后,试验组自我管理中饮食控制、足部护理、高低血糖处理维度得分高于对照组(P<0.05),而运动管理和药物监测维度得分及自我管理总分差异无统计学意义(P>0.05)。干预后,试验组患者的空腹和餐后2h血糖低于对照组(P<0.05),两组糖化血红蛋白差异无统计学意义(P>0.05)。 结论 通过个案管理模式对2型糖尿病患者进行健康管理,可以提高糖尿病患者饮食控制、足部护理和高低血糖处理的自我管理能力,有助于控制其糖代谢指标。

Abstract: Objective To evaluate the effects of case management model in the health management of patients with type 2 diabetes mellitus. Methods Totally 152 diabetic patients were divided into two groups according to the wards of department of endocrinology. Case management model and routine health management model was applied in the experimental group and the control group,respectively. All patients were investigated using the self-manage-ment ability questionnaire before and after intervention. Moreover,the fasting blood glucose,2h postprandial blood glucose and glycosylated hemoglobin were compared between the two groups. Results After intervention,the factor scores of dietary control,foot care and coping with high-low blood glucose management were significantly higher in the experimental group than those of the control group(P<0.05),while no significant differences were found in the total score of self-management and factor scores of sports management and drug monitoring between the two groups (P>0.05). After intervention,the fasting blood glucose and postprandial 2h blood glucose in the experimental group were significantly lower than those of the control group(P<0.05),while no significant difference was found in the glycosylated hemoglobin between the two groups(P>0.05). Conclusion The application of case management model in the health management of patients with type 2 diabetes mellitus can improve the self-management ability in dietary control,foot care and coping with high-low blood glucose,and enhance the control of glucose metabolism.
