中文版高血压患者自我管理评估工具的系统评价

Chinese version of hypertension self-management assessment tools based on COSMIN guidelines:a systematic review

摘要:

目的 系统评价我国高血压患者自我管理评估工具的测量属性及相关研究的方法学质量,为医护人员选择合适的评估工具提供循证支持。 方法 检索中国生物医学文献数据库、中国知网、维普、万方、PubMed、Embase、Web of Science、the Cochrane Library、CINAHL、ProQuest等数据库中有关中文版高血压患者自我管理评估工具测量属性评价的研究,检索时限为建库至2023年5月。由2名研究人员独立进行文献筛选及数据提取,根据基于专家共识的患者报告结局测量工具的遴选指南对评估工具的测量属性及相关研究的方法学质量进行评定,并形成最终推荐意见。 结果 共纳入7项研究,涉及7种中文版高血压患者自我管理评估工具,纳入研究报告的评估工具测量属性仅涉及内容效度、结构效度、内部一致性、稳定性。由于6种评估工具的内容效度均为“不确定”,1项研究未报告评估工具的内容效度,故无A类推荐工具,B类推荐工具有5种,C类推荐工具有2种。仅高血压病人自我管理行为测评量表具有高质量证据证明其内部一致性和结构效度为“充分”。 结论 中文版高血压患者自我管理评估工具多样,但质量整体不高。高血压病人自我管理行为测评量表的结构效度和内部一致性证据质量良好,可暂时被推荐使用,但在其他测量属性方面仍存在不足,未来还需进一步研究完善。

Abstract:

Objective To systematically evaluate the measurement properties of self-management assessment tools for hypertensive patients in China, and to provide evidence-based support for healthcare professionals to select appropriate assessment tools. Methods CBM, CNKI, VIP, Wanfang, PubMed, Embase, Web of Science, the Cochrane Library, CINAHL, and ProQuest were searched to collect studies related to the measurement characteristics evaluation of Chinese version of self-management assessment tools for hypertensive patients. The search time limitation was from the establishment of the database to May 2023. Literature screening and data extraction were performed independently by 2 researchers, who rated the methodological quality and measurement properties of the assessment tools according to the Consensus-Based Standards for the Selection of Health Measurement Instruments (COSMIN) and formed the final recommendations. Results A total of seven studies involving seven Chinese versions of self-management assessment tools for hypertension patients were included, and only content validity, structural validity, internal consistency, and stability were reported. Since the content validity of each assessment tool was uncertain or inadequate, there was no Category A recommended tools, and there were five Category B recommended tools and two Category C recommended tools. There was high-quality evidence to prove that the internal consistency and construct validity of the Hypertension Patients Self-management Behavior Rating Scale were “sufficient”. Conclusion The Chinese version of hypertension self-management assessment tools is diverse, but the overall quality is not high. The construct validity and internal consistency of the Hypertension Patients Self-management Behavior Rating Scale are good, which can be temporarily recommended for use. However, there are still shortcomings in other measurement properties and further research are required in the future.
