摘要

目的 编制适用于测评癌症患者家庭照顾者心理健康素养的情景案例题,并分析其质量。方法 以心理健康素养的概念和内涵为框架,通过文献研究法、专家会议法形成案例题,于2023年7月—2024年4月采用便利抽样法对某三级甲等医院365名癌症患者照顾者进行测评。采用Rasch模型中的题目信度、分离度、单维性检验、怀特图、题目拟合及评分等级结构进行案例题的质量分析。结果 形成1个焦虑案例、1个抑郁案例,共包括8个单选题、3个多选题和8个开放式题目。癌症患者照顾者心理健康素养案例题符合单维性假设,大多数题目拟合较好,题目难度分布与被试能力的分布相近。题目分离度为10.32,题目信度为0.99。结论 癌症患者家庭照顾者心理健康素养案例题与Rasch模型拟合较好,案例题具有可靠性,适用于癌症照顾者心理健康素养水平的测量。

Abstract

Objective To develop case vignettes on mental health literacy for family caregivers of cancer patients in accordance with Chinese regional culture and analyze their quality. Methods Using the concept and connotation of mental health literacy as the framework,the case questions were formed through literature research and expert meeting methods. From July 2023 to April 2024,a convenience sampling method was used to evaluate 365 family caregivers of cancer patients in a tertiary hospital. Item reliability,separation,unidimensionality test,wright map,item fit,and structure measures at intersections of the Rasch model were used to analyze the quality of the case. Results The formal case consists of two scenarios anxiety and depression,including eight single-choice questions,three multiple-choice questions,and eight open-ended questions. The mental health literacy case for caregivers of cancer patients met the unidimensionnlity assumption,with most items fitting well. The item difficulty of the case was compatible with caregivers with an average ability level. The case separation was 10.32,and the item reliability was 0.99. Conclusion The mental health literacy cases of cancer patient caregivers fit well with the Rasch model,and have good reliability and validity,which was suitable for measuring the level of mental health literacy of cancer caregivers.
